早期食管癌及癌前病变内镜黏膜下剥离术后复发的相关危险因素分析

摘　　要：目的探讨早期食管癌及癌前病变内镜黏膜下剥离术(ESD)后复发的相关危险因素.方法选取2017年11月至2021年6月浙江中医药大学附属第三医院消化内科收治的79例食管ESD后的早期食管癌及癌前病变患者的临床资料.分为未复发组及复发组,分析ESD术后复发的相关危险因素.结果79例患者中有12例出现了术后复发.单因素分析显示:多发病灶与ESD术后复发相关,差异有统计学意义(P<0.05).患者的性别、年龄、饮酒史、病变部位、切除长径、切除面积、病理类型、浸润深度与术后复发无关,差异无统计学意义(P>0.05).多因素分析显示:多发病灶将增加ESD术后复发的风险,具有统计学意义(P<0.05).结论多发病灶可能是食管早癌及癌前病变ESD术后复发的独立危险因素.Objective To investigate the risk factors of recurrence after endoscopic submucosal dissection(ESD)for early esophageal cancer and precancerous lesions.Methods The clinical data of 79 patients with early esophageal cancer and precancerous lesions after esophageal ESD who were admitted to the Department of Gastroenterology at the Third Affiliated Hospital of Zhejiang University of Traditional Chinese Medicine from November 2017 to June 2021 were divided into non-recurrence group and recurrence group.The risk factors of recurrence for superficial esophageal cancer and precancerous lesions after ESD were analyzed.Results Recurrence occurred in 12 of 79 patients.Univariate analysis showed that multiple lesions were associated with the recurrence of superficial esophageal cancer and precancerous lesions after ESD(P<0.05).The gender,age,smoking habit,alcohol habit,lesion location,lesion length,lesion area,histopathology,and depth of invasion were not associated with the recurrence(P>0.05).Multivariate analysis showed that multiple lesions would increase the risk of early esophageal cancer recurrence after ESD,with statistical significance(P<0.05).Conclusion Multiple lesions may be an independent risk factor for the recurrence of superficial esophageal cancer and precancerous lesions after ESD.
