上颌骨种植失败影响因素回顾性分析

摘　　要：目的回顾性研究分析可能影响上颌骨内种植体失败的风险因素,以期进一步提高种植成功率.方法收集2016年1月至2019年12月,共1,293颗种植体,809例患者信息,基于Cox回归分析将性别、年龄、种植体位置、直径、长度、附加手术、种植体品牌与患者吸烟习惯纳入预测因素.结果随访期内种植体失败共计25颗,其中早期失败8颗,晚期失败17颗,累积生存率98.07%.多因素Cox回归分析显示:男性失败率显著更高(P<0.05);老年组失败率显著较高(P<0.05);上颌骨不同区域失败率无显著差异;不同直径种植体失败率无显著差异;常规长度种植体失败率显著更低(P<0.05);上颌窦内提升术失败率显著更高(P<0.05);Straumann品牌的失败率显著更低(P<0.05);重度吸烟者的失败率显著提升(P<0.05).结论男性、老年患者、使用短种植体、上颌窦内提升术、采用Megagen EZ Plus品牌及重度吸烟者上颌骨种植失败率显著较高.Objective To analyze the risk factors of implant failure in maxilla,and to improve the implant success rate.Methods From January 2016 to December 2019,1293 implants and 809 patients'information were collected.Cox regression analysis was used to analyze factors including sex,age,implant location,diameter,length,additional surgery,implant brand and smoking habit.Results There were 25 implant failures during the follow-up period,including 8 early failures and 17 late failures.The cumulative survival rate was 98.07%.Multivariate Cox regression analysis showed that the failure rate was significantly higher in men(P<0.05)and in the elderly(P<0.05).There was no significant difference in the failure rate of implants with different diameters.The failure rate of implants with conventional length was significantly lower(P<0.05).The failure rate of intramaxllary sinus augmentation was significantly higher(P<0.05).The failure rate of Straumann brand was significantly lower(P<0.05).The failure rate of heavy smokers was significantly increased(P<0.05).Conclusion The rate of maxillary implant failure is significantly higher in men,older patients,those using short implants,maxillary sinus augmentation,Megeagen EZ Plus and heavy smokers.
