老年人血管性痴呆患病率及危险因素调查

摘　　要：目的调查老年人血管性痴呆(VaD)患病率及危险因素.方法采用多阶段分层整群抽样的方法,抽取60岁及以上的共7,300人作为研究对象,采用自制问卷对其年龄、文化水平等一般资料和糖尿病史、脑卒中病史、高血压病史、吸烟饮酒习惯调查,采用简易精神状态量表(MMSE)进行认知功能评定,根据精神障碍诊断和统计手册第五版(DSM-Ⅴ)确诊VaD;采用单因素及多因素Logistic回归分析VaD的危险因素.结果共有7,246人完成调查问卷,完成调查率为99.3%(7,246/7,300);VaD患者有39例,患病率为0.538%(39/7,246).高龄(OR=2.63)、脑卒中(OR=3.96)、糖尿病(OR=4.01)、高血压(OR=4.32)、抽烟(OR=2.47)是VaD患病的危险因素.结论60岁及以上人群VaD患病率为0.538%;高龄、糖尿病、脑卒中、高血压和吸烟是其危险因素,对其危险因素进行积极干预十分必要.Objective To investigate the prevalence and risk factors of vascular dementia(VaD)in the elderly in Quzhou City.Methods A multistage stratified cluster sampling method was used to select 7,300 people over 60 years old in Quzhou City.A self-made questionnaire was used to investigate their age,educational level and other general information,including history of diabetes,stroke and hypertension,and habits of smoking and drinking.Mini-Mental State Examination(MMSE)was used to assess the cognitive function and the diagnosis of VaD was based on the Diagnostic and Statistical Manual of Mental Disorders,5th Edition(DSM-V).Univariate and multivariate Logistic regression were used to analyze risk factors of VaD.Results There were 7,246 subjects who completed the questionnaire and the survey completion rate was 99.3%(7,246/7,300).There were 39 patients with VaD.The prevalence rate was 0.538%(39/7,246).Aged(OR=2.63),stroke(OR=3.96),diabetes(OR=4.01),hypertension(OR=4.32)and smoking(OR=2.47)were risk factors for VaD.Conclusion The prevalence of VaD among aged 60 and above is 0.538% in Quzhou City.Aged,diabetes,stroke,hypertension and smoking are risk factors forVaD.It is necessary to intervene these risk factors.
