海南省五家住培基地麻醉超声引导下神经阻滞教学问卷调查

摘　　要：目的了解麻醉住培医生的超声引导下神经阻滞掌握情况,并分析影响能力提高有关因素,为后续麻醉教学开展提供参考意见。方法2020年8—10月选择海南省五家住培基地的58名麻醉住培医生为问卷调查对象,发放超声引导下神经阻滞问卷。结果本次调查结果显示,麻醉科住培医生既往操作例数>20的有7人,占总人数12.07%(7/58);腹横肌平面阻滞掌握最好,熟悉及以上人数占比为48.28%(28/58),腹直肌鞘阻滞的熟悉及以上人数占比为43.10%(25/58);住培期间缺少规范化系统性超声培训课程的住培医生、本科及研究生学习阶段没有接触过超声麻醉操作方面知识的住培医生、超声设备少且个人操作机会少分别占比84.48%(49/58)、70.69%(41/58)、65.51%(38/58);合理安排操作机会、建立系统超声神经阻滞教学课程、操作后进行归纳总结提高学习效率选择人数最多,分别占比为93.10%(54/58)、87.93%(51/58)、77.59%(45/58)。结论现阶段麻醉超声教学及实践培训重视程度不足,且培训与实践系统尚不完善,应当加强对于住培医生的培训力度,并通过解决培训教学过程中的难点,持续完善培训教学系统,提供教学培训质量。Objective To understand the mastery of ultrasound-guided nerve block by anesthesiologists in residence,and to analyze the factors affecting the improvement of ability,so as to provide reference for the follow-up of anesthesia teaching.Methods From August 2020 to October 2020,a total of 58 anesthesiologists from five resident standardization training bases in Hainan Province were selected as the subjects of the questionnaire,and the ultrasound guided nerve block questionnaire was issued.Results The results of this survey showed that there were 7 residents of the anesthesia department who had operated more than 20 cases in the past,accounting for 12.07%(7/58)of the total number.The transverse abdominal muscle block was the best,48.28%(28/58)of them were familiar with it,and 43.10%(25/58)of them were familiar with it.During the regular training,the regular training doctors who lack standardized and systematic ultrasonic training courses,the regular training doctors who had not been exposed to the knowledge of ultrasonic anesthesia operation at the undergraduate and postgraduate learning stages,and the regular training doctors who had less ultrasonic equipment and less personal operation opportunities accounted for 84.48%(49/58),70.69%(41/58),and 65.51%(38/58)respectively.The most people chose to reasonably arrange operation opportunities,establish a systematic ultrasonic nerve block teaching course,and conduct induction and summary after operation to improve learning efficiency,which accounted for 93.10%(54/58),87.93%(51/58)and 77.59%(45/58),respectively.Conclusion The degree of attention in the teaching and practice training of anesthesia ultrasound is insufficient at the present stage,and the training and practice system is not perfect.The training for resident doctors should be strengthened,and the teaching quality should be provided by solving the difficulties in the training and teaching process,and continuously improving the training and teaching system.
