摘  要:目的:确定坐骨神经慢性压迫性损伤(CCI)所致神经病理性痛(NP)是否伴有脊髓背角内有髓或无髓神经纤维长度密度或体积分数的变化,以及神经生长因子(NGF)干预的作用。方法:3月龄雄性SD大鼠随机分为sham组、CCI组和CCI+NGF组,术后28 d取材,将每只大鼠左侧和右侧L5脊髓背角各分成4个组织块并分别制成1张各向同性Epon812环氧树脂包埋超薄切片,利用透射电镜(TEM)在每张超薄切片内等距随机抽选视野拍照后,采用体视学方法计算脊髓背角内有髓神经纤维和无髓神经纤维的体积分数、长度密度以及有髓神经纤维的直径及其髓鞘厚度。结果:与未手术侧比较,三组大鼠手术侧L5节段脊髓背角内有髓神经纤维或无髓神经纤维的体积分数或长度密度以及有髓神经纤维的直径或髓鞘厚度之间的差异均无统计学意义,三组大鼠相同侧别的组间比较其差异也无统计学意义。结论:CCI所致NP以及NGF干预不会引起脊髓背角内有髓神经纤维和无髓神经纤维总体积或总长度的改变。Objective: To determine whether peripheral neuropathic pain(NP) induced by chronic constriction injury(CCI) is accompanied by changes in the length density or volume fraction of myelinated or unmyelinated nerve fibers in the rat spinal dorsal horn and the effects of nerve growth factor(NGF) on it. Methods: Three months old male Sprague-Dawley rats were randomly divided into sham group, CCI group and CCI+NGF group. Twenty-eight days after operation, L5segment of the spinal cord of each animal was removed, and then, the left and right spinal dorsal horn were divided into four tissue blocks respectively, and one isotropic Epon812-embedded ultra-thin section was respectively cut from each tissue block. The testing fields were equally spaced sampled and taken photos from each section with transmission electron microscope(TEM), the volume fraction or length density of the myelinated or unmyelinated fibers and the diameter and myelin sheath thickness of myelinated fibers in spinal dorsal horn were estimated with the stereological methods. Results: Compared with the non-operative side, the volume fraction or length density of myelinated nerve fibers or unmyelinated nerve fibers, and the diameter or myelin sheath thickness of myelinated nerve fibers in the spinal dorsal horn of L5segment on the operative side of the three groups were not statistically significant, and also, there was no significant difference on the same side among three groups. Conclusion: CCI-induced NP and NGF intervention do not lead to significant changes in the total volume or length of myelinated and unmyelinated nerve fibers in the spinal dorsal horn.
