摘　　要：目的探讨不同年龄人群的乳腺密度与乳腺癌发病风险的相关性。方法回顾性分析行乳腺X线摄影检查并经病理确诊的乳腺癌患者1941例,以及相同时间内行相同设备检查无阳性发现的门诊体检患者1503例,分别作为实验组和对照组。所有患者的乳腺X线摄影原始图像经Quantra软件处理自动测量乳腺密度。采用Pearson相关性检验分析乳腺密度与年龄的相关性。采用Wilcoxon检验分析在<35岁、35~44岁、45~54岁、55~64岁以及≥65岁的女性人群中乳腺癌患者与正常人群间乳腺密度的差异。结果随年龄增长,乳腺密度逐渐降低(r=-0.39,P<0.05)。在<35岁和35~44岁组的女性中,乳腺癌患者的乳腺密度高于正常人群(P<0.01)。在45~54岁和55~64岁组的女性中,乳腺癌患者的乳腺密度低于正常人群(P<0.05)。而在≥65岁组的女性中两者差异无统计学意义(P=0.74)。结论乳腺密度与乳腺癌发病风险的关系随年龄而改变,对于年轻患者影响较显著。Objective To analyze the relationship between breast density and breast cancer risk in different age groups.Methods Retrospective analysis of 1941 cases of breast cancer diagnosed by mammography(MG)and confirmed by pathology,and 1503 physical examination patients who underwent the same equipment examination in the same time and found no positive findings,respectively as the experimental group and the control group.The original mammographic images of all patients were processed by Quantra to automatically measure the breast density.Pearson correlation test was used to analyze the relationship between age and breast density.Wilcoxon test was used to evaluate the difference of breast density between the experimental group and the control group in women aged<35 years,35-44 years,45-54 years,55-64 years and≥65 years.Results Breast density decreased with age(r=-0.39,P<0.05).In women<35 and 35-44 years old,the breast density was higher in breast cancer patients than in normal population(P<0.01).Among women aged 45-54 and 55-64,the breast density of breast cancer patients was lower than that of the normal population(P<0.05).There was no significant difference between the two groups in women aged≥65 years(P=0.74).Conclusion The relationship between breast density and the risk of breast cancer changes with age,which is more significant for young patients.
