摘　　要：目的分析外周血Bax、Caspase-3蛋白表达水平与急性有机磷农药中毒(Acute organophosphorus pesticide poisoning,AOPP)患者肝损伤的相关性。方法取2018年9月至2020年7月期间于皖南医学院弋矶山医院急诊科收治的AOPP患者145例作为研究对象,并根据《职业性急性有机磷农药中毒诊断标准及处理原则》中制定的病情分级标准,将研究对象分为重度组(n=56例)、中度组(n=50例)以及轻度组(n=39例)。比较各组对象在入院时、入院第1天、3天及7天时外周血Bax、Caspase-3蛋白表达及肝功能指标[清蛋白(ALB)、总胆红素(TBL)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总蛋白(TP)],采用Pearson分析外周血Bax、Caspase-3蛋白表达水平与肝损伤的相关性。结果AOPP患者外周血Bax、Caspase-3表达均为重度组>中度组>轻度组,于不同时间点数值比较差异均有统计学意义(P<0.05),且整体趋势均为先升高后下降。结论AOPP患者外周血Bax、Caspase-3表达与其中毒程度、中毒时间和早期肝损伤密切相关,动态观察外周血Bax、Caspase-3表达水平有利于及时发现患者早期肝功能变化。Objective To analyze the correlation between the expression levels of Bax and Caspase-3 in peripheral blood and liver injury in patients with acute organophosphorus pesticide poisoning(AOPP).Methods A total of 145 AOPP patients admitted to the Department of Emergency Medicine of the first affiliate hospital of Wannan Medical College from September 2018 to July 2020 were selected as the research objects,and the disease classification standards formulated in the"Diagnostic Criteria and Principles of Treatment of Occupational Acute Organophosphorus Pesticide Poisoning".The subjects were divided into severe group(n=56 cases),moderate group(n=50 cases)and mild group(n=39 cases).The expressions of Bax and Caspase-3 in peripheral blood,and the liver function indexes[albumin(ALB),total bilirubin(TBL),alanine aminotransferase(ALT),Aspartate aminotransferase(AST),total protein(TP)]of subjects in each group were compared at admission,day 1,day 3,and day 7 after admission.Pearson analysis was used to analyze the correlation between the expression levels of Bax and Caspase-3 in peripheral blood and liver injury.Results The expression of Bax and Caspase-3 in the peripheral blood of AOPP patients decreased successively in the severe,moderate,and mild groups,and the differences in values at different time points were statistically significant(P<0.05),and the overall trend was increased first and then decreased.Conclusion The expression of Bax and Caspase-3 in the peripheral blood of AOPP patients is closely related to the degree of poisoning,the time of poisoning and the early liver injury.Dynamic observation of the expression of Bax and Caspase-3 in the peripheral blood is helpful for timely detection of early liver function changes in patients.
