一种个性化舌体保护器在近中下颌阻生齿拔除的临床疗效观察

摘　　要：目的观察个性化舌体保护器在近中下颌阻生齿的拔除中,防止高速涡轮手机对舌体及软组织损伤的临床疗效。方法回顾性分析2018年5月-2020年4月于甘肃医学院附属医院口腔科就诊、符合纳入标准的300例近中下颌阻生智齿拔除患者的临床资料。按照门诊挂号顺序随机分为实验组和对照组,其中实验组联合使用高速涡轮微创手机与舌体保护器,对照组单独使用高速涡轮微创手机。按照实验预设患者复诊时间,详细记录患者术后各种临床反应指标,比较分析两组临床疗效及不良反应的发生率并进行统计学分析。结果实验组与对照组软组织损伤阳性例数分别为0例、34例,差异有统计学意义(P<0.001);实验组张口受限发生率明显小于对照组,差异有统计学意义(P<0.001)。实验组与对照组干槽症的阳性例数分别为1例、24例,差异有统计学意义(P<0.001)。术后观察结果显示,实验组术中疼痛程度发生率、肿胀发生率及拔牙时间明显小于对照组,差异有统计学意义(P<0.001)。结论近中下颌高位阻生齿用高速仰角手机截冠时,高速转动的手机长裂钻常会伤及舌腹黏膜。舌体保护器将舌体组织置于口底,不会伤及舌腹黏膜,缩短手术时间,减轻术后并发症。Objective To observe clinical effect of individualized tongue protector in extraction of mandibular mesial impacted teeth to prevent tongue and soft tissue injury caused by highspeed turbine handpieces.Methods The paper analyzed reviewed clinical data of 300 patients with mandibular impacted wisdom teeth extraction meeting inclusion criteria from May 2018 to April 2020 in stomatology department of Gansu Medical College Affiliated Hospital,and randomly divided them into experimental group and control group according to outpatient registration order.Experimental group was treated high-speed turbine minimally invasive mobile phones and tongue protectors jointly,and control group with high-speed turbine minimally invasive mobile phones only.According to preset time of patient's return visit in experiment,various clinical response indicators of patients after surgery were recorded in detail,and clinical effect and incidence of adverse reactions between two groups was compared and analyzed statistically.Results Positive cases of soft tissue injury in experimental group and control group were 0 and 34 respectively,with statistically significant difference(P<0.001).Incidence of mouth opening limitation in experimental group was significantly lower than control group(P<0.001).Number of positive cases of dry socket in experimental group and control group was 1 and 24 respectively,with statistically significant difference(P<0.001).Postoperative observation results showed that incidence of pain,swelling and extraction time in experimental group was significantly lower than control group(P<0.001).Conclusion Lingual and abdominal mucosa may often be injured by long split drill with high speed rotation during crown of impacted mandibular proximal middle and high teeth being cut by high speed elevation hand piece.Tongue protector may place tongue tissue at bottom of the mouth,not hurting tongue and abdominal mucosa,which can shorten operation time and reduce postoperative complications.
