基于中医传承辅助系统研究牛学恩辨证治疗功能性胃肠病伴抑郁用药规律

摘　　要：目的利用中医传承辅助平台(V2.5)软件探究牛学恩教授临证诊疗功能性胃肠病伴抑郁的治疗规律。方法收录牛学恩教授门诊诊疗功能性胃肠病伴抑郁的获效方剂,对中药使用频次、用药模式、新处方等采用改进的互信息法、复杂系统熵聚类分析等实行软件分析。结果共筛选出127张处方,177味中药,四气统计得出温性药占42.10%、寒性药占26.66%,两者总占68.76%。中药使用频次居前10位的药物依次为茯苓、甘草、半夏、柴胡、白芍、白术、当归、陈皮、生姜、黄芩。同时得出牛学恩教授临证治疗该病的组方规律网格图。最后,通过复杂系统熵聚类分析得出“竹茹-枳实-莲子心-灯心草”“海螵蛸-焦神曲-夏枯草-青皮-龙胆草-胡黄连”“党参-枳壳-白术-枳实”等新方核心组合13组。结论牛学恩教授治疗功能性胃肠病伴抑郁时,寒温并用各行其是,补虚泻实随证用之,重视调节中焦气机、升降左右,并结合疏肝、健脾、温阳、通下等法,尤善用辛开苦降之法使气机升降有度,脏腑协调互用,气机枢转有度,则阴阳和,心神安、胃肠治、抑郁消。Objective To explore the medication rules of Professor Niu Xue-en in the clinical diagnosis and treatment of functional gastrointestinal disorders with depression by using the Traditional Chinese Medicine Inheritance Support System(V2.5).Methods The prescriptions prescribed by Professor Niu Xue′en in the treatment of functional gastrointestinal disorders with depression in the outpatient department were selected for study.The improved mutual information method and complex system entropy clustering analysis were employed to study the use frequency,medication pattern,and new prescriptions of Chinese medicine.Results A total of 127 prescriptions were screened out,involving 177 Chinese herbs.The percentages of warm drugs and cold drugs were 42.10%and 26.66%,respectively,with the sum of 68.76%.The top 10 Chinese herbs with high use frequency were Poria,Glycyrrhizae Radix et Rhizoma,Pinelliae Rhizoma,Bupleuri Radix,Paeoniae Radix Alba,Atractylodis Macrocephalae Rhizoma,Angelicae Sinensis Radix,Citri Reticulatae Pericarpium,Zingiberis Rhizoma Recens,and Scutellariae Radix.At the same time,we obtained the grid diagram of the medication rules of Professor Niu Xue-en in the clinical treatment of this disease.Finally,the complex system entropy cluster analysis revealed 13 new key combinations such as“Bambusae Caulis in Taenias-Aurantii Fructus Immaturus-Nelumbinis Plumula-Junci Medulla”,“Sepiae Endoconcha-Massa Medicata Fermentata-Prunellae Spica-Citri Reticulatae Pericarpium Viride-Gentianae Radix et Rhizome-Picrorhizae Rhizoma”,and“Codonopsis Radix-Aurantii Fructus-Atractylodis Macrocephalae Rhizoma-Aurantii Fructus Immaturus”.Conclusion Professor Niu Xue-en treats functional gastrointestinal disorders with depression by combining cold and warm drugs,tonifying deficiency and purging excess,regulating qi in the middle energizer,ascending and descending.In addition,he employs the methods of soothing liver,invigorating spleen,warming yang,and purgative method.He is especially good at regulating qi movement and
