扶正减毒方联合GLF方案治疗晚期食管癌患者的疗效及对其血清碱性成纤维细胞生长因子、胰岛素样生长因子-1水平的影响

摘　　要：目的 探讨扶正减毒方联合GLF方案治疗晚期食管癌的疗效及对血清碱性成纤维细胞生长因子(Basic fibroblast growth factor, bFGF)、胰岛素样生长因子-1(Insulin-like growth factor-1,IGF-1)水平的影响。方法 选取2016年1月—2021年11月期间河南省郑州市中医院收治的晚期食管癌患者87例,采用随机数字表法分为对照组42例和治疗组45例。对照组采用GLF方案化疗,治疗组在对照组基础上采用扶正减毒方治疗,3周为1个周期,用药至疾病进展或毒性不能耐受,最多治疗6个周期。观察比较两组患者临床疗效及毒副作用发生率,血清bFGF、IGF-1水平的变化及生活质量改善稳定率。结果 治疗后治疗组总有效率64.44%(29/45)高于对照组40.48%(17/42),差异有统计学意义(P<0.05)。治疗后两组患者血清bFGF、IGF-1水平较治疗前降低,差异有统计学意义(P<0.05);且治疗组血清bFGF、IGF-1水平低于对照组,差异有统计学意义(P<0.05)。治疗后治疗组生活质量改善稳定率71.11%(32/45)高于对照组50.00%(21/42),差异有统计学意义(P<0.05)。治疗后治疗组毒副作用发生率4.44%(2/45)低于对照组23.81%(10/42),差异有统计学意义(P<0.05)。结论 扶正减毒方联合GLF化疗治疗晚期食管癌的疗效显著,能明显改善患者生活质量,降低血清bFGF、IGF-1水平,减少化疗毒副作用。Objective To observe the therapeutic effect of Fuzheng Jiandu Formula combined with gemcitabine plus 5-fluorouracil/leucovorin(GLF)regimen in the treatment of advanced esophageal cancer and the influence of this therapy on the serum levels of basic fibroblast growth factor(bFGF)and insulin-like growth factor-1(IGF-1).Methods A total of 87 patients with advanced esophageal cancer treated in Zhengzhou Hospital of Traditional Chinese Medicine from January 2016 to November 2021 were assigned into two groups by the random number table method.The 42 patients in the control group were treated with GLF chemotherapy, and the 45 patients in the observation group were additionally treated with Fuzheng Jiandu Formula.The treatment lasted for at most six courses(each of three weeks)until the disease progressed or the toxicity was intolerable.The clinical efficacy and incidence of adverse reactions were compared between the two groups.The changes in the serum levels of bFGF and IGF-1 before and after treatment were measured.The quality of life(QOL)was assessed based on the Karnofsky Performance Scale(KPS)score.Results The overall response rate of the observation group(64.44%,29/45)was higher than that(40.48%,17/42)of the control group(P<0.05).After treatment, the serum levels of bFGF and IGF-1 declined compared with those before treatment(P<0.05),and the serum levels of bFGF and IGF-1 in the observation group were lower than those in the control group(P<0.05).The improvement and stability rate of QOL in the observation group was 71.11%(32/45),which was higher than that(50.00%,21/42)in the control group(P<0.05).The incidence of adverse reactions in the observation group was 4.44%(2/45),which was lower than that(23.81%,10/42)in the control group(P<0.05).Conclusion Fuzheng Jiandu Formula combined with GLF chemotherapy has significant efficacy in the treatment of advanced esophageal cancer, which can significantly improve the QOL,lower the serum levels of bFGF and IGF-1,and reduce the toxic and side effects of chemotherapy on the
