基于玄府理论探讨慢性酒精中毒性周围神经病的诊治思路

摘　　要：慢性酒精中毒性周围神经病(Chronic Alcoholism Peripheral Neuropathy,CAPN)可归属于中医学“痹证”“痿证”范畴,其发病隐匿、病程漫长,常累及肢体远端感觉及运动神经,产生疼痛、感觉异常等表现,严重者出现慢性无力和肌萎缩,最终肌肉、神经失养,大肉渐脱而萎缩。文中基于玄府理论,系统阐述玄府与CAPN的相关性,概括玄府致病的主要病机为津精不布、气机不畅、血运不通。并从开通玄府、恢复玄府正常功能的角度论述治疗CAPN可将祛湿开玄、泄热复通,调气开玄,活血复通作为基本治法。以期为临床治疗CAPN开拓新思路,进一步拓展玄府理论的应用范围。Chronic alcoholism peripheral neuropathy(CAPN),categorized as“arthralgia”or“atrophy”in traditional Chinese medicine,features insidious onset and long course.It affects thesensation of distallimbs and motor nerve,resulting in pain and abnormal sensation.In severe cases,chronic weakness and muscular atrophy occur,and eventually the muscles and nerves are deprived of nourishment,leading to the gradual loss offlesh and atrophy.Based on the theory of sweat pore,this article systematically describes the correlation between sweat pore and CAPN,and summarizes that the disorders following pore obstruction mainly result from failure to distribute fluid and essence,disturbance of qi movement,and block of blood flow.In addition,this study also discusses the treatment of this disease from the perspective of unblocking sweat pore and restoring its normal function by dispelling dampness,unblocking sweat pore,discharging heat and invigorating blood,and regulating qi and activating blood.Moreover,unblocking the sweat pore and restoring blood circulation should be the basic treatment principle.This study is expected to provide new ideas for the treatment of CAPN and extend the application of the sweat pore theory.
