人乳头瘤病毒疫苗接种不良反应的主动监测与全程接种率的提高对策

摘　　要：目的探究人乳头瘤病毒(HPV)疫苗接种不良反应,并调查未及时全程接种的原因。方法选取2020年在南京医科大学附属江宁医院预防接种门诊的1242名HPV疫苗受种者作为研究对象。主动监测接种不良反应,通过医疗信息系统查询未及时全程接种者,电话随访核实未继续接种的原因。结果1242名女性接种后30 min内无局部和全身疑似不良反应,接种2周后局部反应常见为轻微酸胀、肿胀疼痛;全身反应有疲乏、头晕头痛。HPV疫苗接种后疑似不良反应发生率为55.56%,严重接种不良反应发生率为0.40%。同年应完成全程接种的1758名受种女性中,有54名未完成全程接种,未及时全程接种率为3.07%。其中因学习和工作忙而遗忘者占61.11%、因意外妊娠者占22.22%、因不能耐受接种不良反应及中途行腹部手术者占5.56%。结论疫苗安全性是未及时全程接种的重要因素之一,及时提醒后续接种时间、严格把握接种适应证和禁忌证,有利于提高全程接种率。Objective To investigate the adverse reactions of human papilloma virus(HPV)vaccination and the reasons for not timely and fully vaccinated.Methods A total of 1242 HPV vaccine recipients in the vaccination clinic of the Affiliated Jiangning Hospital of Nanjing Medical University in 2020 were selected as the research subjects.Active monitoring of adverse reactions to vaccination,and the reasons for non-continuation of vaccination were verified through telephone follow-up.Results There were no local and systemic adverse reactions in 1242 women within 30 min after vaccination,and the most common local reactions were mild acid bilge and swelling pain after 2 weeks of vaccination.Systemic reactions included fatigue,dizziness and headache.After HPV vaccination,the incidence of suspected adverse reactions was 55.56%,and the incidence of serious adverse reactions was 0.40%.Among 1758 women who should complete the whole vaccination in the same year,54 women did not complete the vaccination,and the rate of non-timely full vaccination was 3.07%.Among them,61.11%forgot because of busy study and work,22.22%because of accidental pregnancy,5.56%because they could not tolerate adverse reactions of vaccination and had abdominal surgery during the course.Conclusion The safety of vaccination is one of the important factors of non-timely full vaccination.Timely reminding of the follow-up vaccination time and strictly grasping the indications and contraindications of vaccination are conducive to improving the vaccination rate.
