健脾解毒汤联合肝动脉化疗栓塞术治疗原发性肝癌临床研究

摘　　要：目的:观察健脾解毒汤联合肝动脉化疗栓塞术(TACE)治疗原发性肝癌(PHC)的临床疗效及对患者生活质量的影响。方法:采用随机数字表法将86例PHC患者分为治疗组与对照组各43例。对照组给予TACE治疗,治疗组在对照组基础上运用健脾解毒汤治疗。比较2组临床疗效及不良反应发生率,观察2组治疗前后血清基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)、血管内皮生长因子(VEGF)、白细胞介素-2(IL-2)水平及生活质量评分的变化。结果:治疗组总缓解率51.16%,高于对照组30.23%(P<0.05)。治疗后,2组血清MMP-2、MMP-9、VEGF、IL-2水平均较治疗前降低(P<0.05),治疗组血清MMP-2、MMP-9、VEGF、IL-2水平均低于对照组(P<0.05)。治疗后,2组心理功能、社会关系、生理功能和物质生活评分均较治疗前升高(P<0.05),治疗组上述评分均高于对照组(P<0.05)。治疗组不良反应发生率4.65%,低于对照组18.60%(P<0.05)。结论:健脾解毒汤联合TACE治疗PHC疗效显著,能有效调节患者血清MMP-2、MMP-9、VEGF、IL-2水平,改善其生活质量,降低不良反应发生率。Objective:To observe the clinical effect of Jianpi Jiedu Decoction combined with transarterial chemoembolization(TACE)for primary hepatic cancer(PHC)and its effect on quality of life.Methods:A total of 86 cases of PHC patients were divided into the treatment group and the control group according to the random number table method,with 43 cases in each group.The control group was treated with TACE,and the treatment group was additionally treated with Jianpi Jiedu Decoction based on the treatment of the control group.The clinical effects and the incidence of adverse reactions in the two groups were compared.Before and after treatment,the changes in levels of matrix metalloproteinase-2(MMP-2),matrix metalloproteinase-9(MMP-9),vascular endothelial growth factor(VEGF)and interleukin-2(IL-2),and scores of quality of life were observed between the two groups.Results:The overall response rate was 51.16% in the treatment group,higher than that of 30.23% in the control group(P<0.05).After treatment,the levels of MMP-2,MMP-9,VEGF and IL-2 in serum in the two groups were decreased when compared with those before treatment(P<0.05),the above levels in serum in the treatment group were lower than those in the control group(P<0.05).After treatment,the scores of mental function,social relations,physiological function and material life in the two groups were increased when compared with those before treatment,and which were higher in the treatment group than those in the control group(P<0.05).The incidence of adverse reactions was 4.65% in the treatment group,lower than that of 18.60% in the control group(P<0.05).Conclusion:Jianpi Jiedu Decoction combined with TACE has a significant curative effect in treating PHC,and can effectively regulate the levels of MMP-2,MMP-9,VEGF and IL-2,improve quality of life of patients,and reduce the incidence of adverse reactions.
