摘　　要：目的探讨腹腔镜阑尾切除术对急性阑尾炎患者术后疼痛程度、免疫应激反应及并发症的影响。方法136例急性阑尾炎患者根据手术方案的不同分为对照组(开腹阑尾切除术)和观察组(腹腔镜阑尾切除术),比较两组的术后疼痛程度、免疫应激反应及并发症发生率。结果观察组术后1d、3d的VAS评分均低于对照组,术后并发症发生率低于对照组(P<0.05)。术后3d,两组的IgM、IgG、PA水平均低于术前,但观察组的血清IgM、IgG、PA水平均高于对照组(P<0.05)。结论与开腹阑尾切除术相比,腹腔镜阑尾切除术可明显减轻急性阑尾炎患者的术后疼痛程度及免疫应激反应,降低术后并发症发生率。Objective To explore the impact of laparoscopic appendectomy on postoperative pain degree,immune stress response and complications in patients with acute appendicitis.Methods 136 patients with acute appendicitis were divided into the control group(open appendectomy)and the observation group(laparoscopic appendectomy)according to different surgical schemes.The postoperative pain degree,immune stress response and incidence of complications were compared between the two groups.Results The VAS scores at 1 d and 3 d after surgery of the observation group were lower than those of the control group,and the incidence of postoperative complications was lower than that of the control group(P<0.05).At 3 days after surgery,the levels of IgM,IgG and PA in both groups were lower than those before surgery,but the levels of serum IgM,IgG and PA of the observation group were higher than those of the control group(P<0.05).Conclusions Compared with open appendectomy,laparoscopic appendectomy can significantly reduce postoperative pain degree and immune stress response in patients with acute appendicitis,and reduce the incidence of postoperative complications.
