摘　　要：目的探讨根尖刮治术与根尖切除术治疗复杂慢性根尖周炎的效果及对患者炎性指标的影响。方法150例复杂慢性根尖周炎患者按照手术治疗方案的不同分为两组,刮治组采用根尖刮治术治疗,切除组采用根尖切除术治疗,比较两组的治疗效果、疼痛程度及炎性指标水平。结果切除组的治疗总有效率为94.67%,高于刮治组的82.67%(P<0.05)。术后1 d、3 d及7 d,两组的VAS评分均低于术前(P<0.05),但组间比较无明显差异(P>0.05)。术后1个月,切除组的TNF-α、IL-1、IL-6水平均低于刮治组(P<0.05)。结论根尖切除术可明显降低复杂慢性根尖周炎患者的术后疼痛程度,改善炎性指标水平。Objective To explore the effects of apical curettage and apical resection in the treatment of complex chronic periapical periodontitis and their impact on inflammatory indicators of patients.Methods 150 patients with complex chronic periapical periodontitis were divided into two groups according to different surgical treatment schemes.The curettage group was treated with apical curettage,and the resection group was treated with apical resection.The therapeutic effect,pain degree and levels of inflammatory indicators were compared between the two groups.Results The total effective rate of treatment in the resection group was 94.67%,higher than 82.67%in the curettage group(P<0.05).At 1 d,3 d and 7 d after surgery,the VAS scores of the two groups were lower than those before surgery(P<0.05),but no significant difference was found between the two groups(P>0.05).At 1 month after surgery,the levels of TNF-α,IL-1 and IL-6 in the resection group were lower than those in the curettage group(P<0.05).Conclusions Apical resection can significantly reduce postoperative pain degree and improve the levels of inflammatory indicators of patients with complex chronic periapical periodontitis.
