摘  要:目的:研究正畸治疗中微种植体支抗压低上颌伸长磨牙对被压低牙齿及牙周软、硬组织的变化。方法:从2018年5月-2021年8月正畸科就诊患者中随机选取40例种植修复前需微种植体支抗压低上颌伸长磨牙的病例。用锥形束CT和佛罗里达Florida电子探针分别测量和观察正畸压低磨牙前、后伸长磨牙牙体长度,颈部牙槽骨高度,牙周探诊深度(probing depth, PD)和改良龈沟出血指数(modified sulcus bleeding index, mBI);利用视觉模拟评分(visual analogue scale, VAS)法评价患者种植修复后的主观满意情况。结果:40颗伸长磨牙均得到有效压低,压低量(2.41±0.53) mm,压低时间为(6.50±1.53)个月。正畸前、后牙体长度分别为(17.77±0.10) mm、(16.38±0.09) mm,颈部牙槽骨高度分别为(2.76±0.11) mm、(3.06±0.13) mm,PD分别为(2.45±0.11) mm、(2.79±0.13) mm,mBI评分分别为(0.69±0.21)分、(0.96±0.22)分,差异均有统计学意义(均P<0.05)。种植体负重3个月后的总体VAS得分为(88.36±9.66)分,其中咀嚼功能得分为(87.64±10.63)分,舒适度得分为(92.60±13.67)分。结论:长期伸长磨牙普遍存在牙周病,微种植体支抗矫治力会导致此类牙周问题的短暂性加重,但合适的矫治力能将其牙根及牙槽骨的吸收控制在较低的水平。Objective: To study the effect of micro implant anchorage on the reduction of maxillary elongated molars in orthodontic treatment, and to compare the changes of tooth body and periodontal soft and hard tissue before and after treatment.Methods: 40 cases of maxillary extended molars requiring micro implant anchorage before implant restoration were randomly selected from the patients treated in Orthodonties Department from May 2018 to August 2021. Cone beam computed tomogra-phy and Florida probe were used to measure and observe the length of anterior and posterior teeth of orthodontic depressed molars, the height of cervical alveolar bone, the depth of periodontal probing(PD), modified sulcus bleeding index(mBI). The subjective satisfaction of patients after implant repair were used visual analog scale(VAS) to evaluate. Results: The elongate molars were all intruded successfully by(2.41±0.53) mm, the average period of orthodontic treatment was(6.50±1.53) months.The length of teeth before and after orthodontic treatment was(17.77 ±0.10) mm and(16.38±0.09) mm, respectively(P<0.05).The height of alveolar bone before and after orthodontic treatment was(2.76 ±0.11) mm and(3.06±0.13) mm, respectively(P<0.05). PD before and after orthodontic treatment were(2.45±0.11) mm and(2.79±0.13) mm, respectively(P<0.05). The mBI score before and after orthodontic treatment were 0.69 ±0.21 and 0.96±0.22, respectively(P<0.05). The overall VAS score after 3months of implant loading was 88.36±9.66, including masticatory function score of 87.64 ±10.63 and comfort of 92.60±13.67.Conclusion: The periodontal problems of long-term elongated teeth are common. The corrective force of micro implant anchorage will lead to the temporary aggravation of such periodontal problems. Reasonable corrective force can control the absorption of cervical alveolar bone and root at a low level after long-term elongation.
