摘 要:目的 分析肺癌相关自身抗体在实性肺结节的良恶性筛查中的临床价值。方法 回顾性分析156例肺癌患者和190例非肺癌患者,均采用酶联免疫吸附试验(ELISA)检测七项相关自身抗体的血清表达水平,通过ROC曲线比较自身抗体单项、多项联合在良恶性筛查中的价值。结果 在所有肺结节患者中,肺癌组患者的七项自身抗体血清表达水平明显高于非肺癌组(P<0.05),ROC曲线显示P53的灵敏度最高(82.7%),CAGE的特异性最高(83.7%),P53联合CAGE的灵敏度为90.9%,AUC为0.730。在实性肺结节患者中,肺癌组患者的七项抗体阳性表达率明显高于非肺癌组,ROC曲线显示P53、MAGEA1的灵敏度最高(77.3%),CAGE的特异性最高(93.0%),P53联合CAGE的灵敏度为84.6%,AUC为0.706。七项相关自身抗体联合检测的灵敏度为95.5%,特异度为69.8%,AUC为0.853。结论 肺癌相关自身抗体P53、CAGE在实性肺结节的良恶性判断中具有较高的敏感性和特异性,可作为早期筛查的参考依据。Objective To analyze the clinical value of lung cancer-related auto-antibodies in the benign and malignant screening of solid pulmonary nodules.Methods Retrospective analysis was performed on 156 patients with lung cancer and 190 patients without lung cancer. Seven related autoantibodies were detected by enzyme-linked immunosorbent assay(ELISA). The value of single and multiple autoantibodies combined in benign and malignant screening was compared by the ROC curve.Results Among all patients with lung nodules, the serum expression levels of the seven auto antibodies were significantly higher in the lung cancer group than in the non-lung cancer group(P<0.05). The ROC curves showed that P53 had the highest sensitivity(82.7%), CAGE had the highest specificity(83.7%), and the sensitivity of P53 combined with CAGE was 90.9%, with an AUC of 0.730. In patients with solid lung nodules, the rate of positive expression of the seven antibodies was significantly higher in the lung cancer group than in the non-lung cancer group. The ROC curves indicated that P53 and MAGEA1 had the highest sensitivity(77.3%), and CAGE had the highest specificity(93.0%). P53 combined with CAGE had an 84.6% sensitivity, with an AUC of 0.706. The combined test for the seven lung tumor-associated autoantibodies had a sensitivity of 95.5%, a specificity of 69.8%, and an AUC of 0.853. Conclusion Lung tumor-associated autoantibodies P53 and CAGE exhibit good sensitivity and specificity in classifying solid lung nodules as benign or malignant. They might be used as a reference basis for early screening.
