Motomed训练及电针在卒中后痉挛性偏瘫中的疗效

摘　　要：目的研究Motomed智能运动训练+低频电针运用于脑卒中后痉挛性偏瘫中的价值。方法将2020年3月—2021年6月厦门大学附属第一医院收住的74例脑卒中后痉挛性偏瘫患者进行分组,以随机列表法划分成两组,分别37例,联合组开展Motomed智能运动训练+低频电针,单一组实施低频电针,两周后对两组总有效率进行评价,两周后评估Ashworth痉挛分级情况,治疗前后进行神经功能缺损(national institute of health stroke scale,NIHSS)、运动功能(Fugl-Meyer)、临床症状积分、生活质量(quality of life,QOL)评分,比较两组治疗结果。结果联合组总有效率89.19%高于单一组67.57%,差异有统计学意义(P<0.05)。联合组Ashworth痉挛0级、1级发生率高出单一组,但2级、3级、4级发生率低于单一组,差异有统计学意义(P<0.05)。治疗前两组NIHSS、Fugl-Meyer评分比较差异无统计学意义(P>0.05),治疗后联合组NIHSS低于单一组,但Fugl-Meyer高出单一组,差异有统计学意义(P<0.05)。治疗前各组临床症状积分、QOL评分差异无统计学意义(P>0.05),联合组治疗后的症状积分低于单一组,但QOL较单一组升高,差异有统计学意义(P<0.05)。结论Motomed智能运动训练+低频电针的效果显著,不仅缓解症状,改善痉挛程度,增强神经功能及运动能力,也对提升生活质量有积极的作用。Objective To evaluate the clinical value of Motomed intelligent sports training plus low-frequency electric acupuncture in post-stroke spastic hemiplegia patients.Methods A total of 74patients with post-stroke spastic hemiplegia treated from March 2020 to June 2021 in the First Affiliated Hospital of Xiamen University were selected.According to random number table,patients were divided into two groups,37 cases in each group.The research group took the Motomed intelligent sports training and low-frequency electric acupuncture;the control group took the low-frequency electric acupuncture.Therapy effects were appraised through the total effective rate,Ashworth spasm grading after two weeks,national institute of health stroke scale(NIHSS),fugl-meyer assessment(FMA),clinical symptom and quality of life(QOL)scores.Results The total effective rate of combination group was 89.19%higher than that of single group(67.57%),and the difference was statistically significant(P<0.05).The incidence of grade 0 and 1 Ashworth spasm in combination group was higher than that in single group,but the incidence of grade 2,3 and 4 Ashworth spasm was lower than that in single group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in NIHSS and Fugl-Meyer scores between the two groups(P>0.05).After treatment,NIHSS in combination group was lower than that in single group,but Fugl-Meyer was higher than that in single group,with statistical significance(P<0.05).Before treatment,there was no significant difference in clinical symptom score and QOL score among all groups(P>0.05).Symptom score of combination group after treatment was lower than that of single group,but QOL was higher than that of single group,and the difference was statistically significant(P<0.05).Conclusion The Motomed intelligent sports training and low-frequency electric acupuncture can reduce the clinical symptoms and spasm degrees,and improve the neurological function,exercise performance and quality of life.
