摘  要:目的探究细胞周期蛋白依赖性激酶1(cyclin-dependent kinase 1,CDK1)和细胞周期蛋白B1(cell cycle-related proteins cyclin B1,CCNB1)在肝细胞癌(hepatocellular carcinoma,HCC)组织中的表达水平及其临床意义。方法采用实时定量聚合酶链反应(qRT-PCR)检测患者HCC肿瘤组织和癌旁正常组织中CDK1与CCNB1的相对表达水平,受试者工作特征(ROC)曲线评估CDK1和CCNB1以及两者并联模型在HCC中的诊断价值以及和病理分期的相关性。随访5年,评估患者CDK1和CCNB1表达水平对患者无进展生存期(progression-free survival,PFS)和总生存期(overall survival,OS)的影响。结果CDK1、CCNB1在HCC组织中表达显著上调,二者间呈正相关(r=0.769,P<0.01),且均与HCC病理分期显著正相关(r=0.828、0.726,P<0.01)。CDK1和CCNB1诊断HCC的曲线下面积(AUC)分别为0.817和0.810,联合诊断的AUC值为0.837。相比于CDK1和CCNB1高表达组,CDK1和CCNB1低表达的HCC患者PFS和OS均显著延长,差异有统计学意义(P<0.05)。结论CDK1、CCNB1在HCC中具有较高的临床诊断和预后价值,两者的表达水平和HCC的病理分期密切相关。Objective To investigate the expression levels of cyclin-dependent kinase 1(CDK1)and cell cycle-related proteins cyclin B1(CCNB1)in hepatocellular carcinoma(HCC)and their clinical significance.Methods The relative expression levels of CDK1 and CCNB1 in HCC tumor tissues and normal adjacent tissues were detected by real-time quantitative polymerase chain reaction(qRT-PCR).The diagnostic value of CDK1 and CCNB1 and their parallel models in HCC and their correlation with pathological stages were evaluated by the receiver operating characteristic(ROC)curve.The effect of CDK1 and CCNB1 expression levels on progression free survival(PFS)and overall survival(OS)was evaluated through 5-year follow-up.Results The expression of CDK1 and CCNB1 was significantly up-regulated in HCC tissues,there was a positive correlation between them(r=0.769,P<0.01),and both were significantly correlated with the pathological stage of HCC(r=0.828,0.726,P<0.01).The area under curve(AUC)values of HCC diagnosed by CDK1 and CCNB1 were 0.817 and 0.810,respectively,and the AUC value of HCC diagnosed by the combination of CDK1 and CCNB1 was 0.837.Compared with the high expression group of CDK1 and CCNB1,the PFS and OS of HCC patients with low expression of CDK1 and CCNB1 were significantly longer,and the difference was statistically significant(P<0.05).Conclusions CDK1 and CCNB1 have high clinical diagnostic and prognostic value in HCC,and their expression levels are closely related to the pathological stages of HCC.
