摘  要:上鼓室是中耳胆脂瘤常见的好发部位。显微镜下胆脂瘤手术为了清除病灶,需要破坏更多结构进行暴露,给修复带来一定的困难,而内镜手术的优点是可以抵近观察,对隐匿的病灶更好的清除,减少对骨性结构的破坏,利于术中重建。随着耳内镜技术的发展,尤其是水下磨骨技术的开展,扩大了耳内镜胆脂瘤手术的适应症。相对于显微镜手术,耳内镜手术需要投入的经济投资更少,因此更多的基层耳科医生热衷于耳内镜手术,而耳内镜手术医生的规范化培训显得非常重要。为了提高中耳上鼓室的手术质量,培训专科医师,需通过耳内镜下中耳上鼓室解剖的教学培训,给耳内镜手术打下基础。本文通过梳理临床工作和手术教学过程中的经验,归纳和总结耳内镜在中耳上鼓室解剖和手术教学中的优势。The attic is a predilection site for cholesteatoma of middle ear.While microscopic cholesteatoma surgery requires the destruction of more structures for exposure in order to remove the lesion,making repair difficult,the advantages of endoscopic surgery are that it allows for closer observation,better removal of hidden lesions,less destruction of bony structures,and facilitates intraoperative reconstruction.With the development of endoscopic techniques,especially underwater osseous grinding,the indications for endoscopic cholesteatoma surgery have been expanded.Compared to microscopic surgery,endoscopic surgery requires less economic investment,so more primary otologists are enthusiastic about endoscopic surgery,and standardized training of endoscopic surgeons becomes very important.In order to improve the quality of surgery in the attic of the middle ear and to train specialists,it is necessary to give a foundation for endoscopic surgery by teaching training in the anatomy of the attic of the middle ear.This paper summarizes the advantages of oto-endoscope in the attic anatomy and operation teaching by combing our experience in clinical work and surgical teaching.
