摘　　要：目的总结并分析经鼻内镜切除鼻内翻性乳头状瘤的要点。方法回顾2015年1月~2020年5月在我科手术治疗的鼻内翻性乳头状瘤患者27例,按Krouse分级标准分为Ⅰ级2例,Ⅱ级5例,Ⅲ级17例,Ⅳ级3例,经鼻内镜手术者20例,鼻内镜联合柯氏入路手术者7例。结果患者随访1-5年,3例复发,其中经鼻内镜手术者2例,鼻内镜联合柯氏入路手术者1例,二者复发率差异无统计学意义,所有患者均未出现眶内及颅内并发症。结论经鼻内镜手术治疗鼻内翻性乳头状瘤安全有效,肿瘤根蒂部位识别和处理非常重要,合适的手术入路、良好的手术器械和娴熟的鼻内镜手术技能是确保手术成功的关键,术后严密随访是降低二次手术的重要手段。Objective To summarize the technique of transnasal endoscopic surgery for nasal inverted papilloma.Method A total of 27 patients with nasal inverted papilloma were treated by the nasal endoscopic surgery,and their data were analyzed retrospectively.Among all patients,the nasal inverted papilloma excision by endoscopy was used in 20 cases,the nasal endoscopy combined with Caldwell-Luc surgery was used in 7 cases.Result All patients were followed up for 1-5 years,and 3 patients relapsed,including 2patients who underwent nasal endoscopic surgery and 1 patient who underwent nasal endoscopic surgery combined with Caldwell-Luc surgery.There was no statistical significance in the recurrence rate between the two groups,and no intraorbital or intracranial complications occurred in all patients.Conclusion Transnasal endoscopic surgery is safe and effective for the treatment of inverted nasal papilloma,and the identification and treatment of the root of the tumor is very important.Appropriate surgical approach,good surgical instruments and skilled nasal endoscopic surgical skills are the key to ensure the success of the operation,and close postoperative follow-up is an important way to reduce the recurrence rate.
