改良通气管路连接在喉癌手术中的应用研究

摘　　要：目的探讨改良通气管路连接法在喉癌手术中的应用价值。方法2018年1月~2020年10月于首都医科大学附属北京友谊医院耳鼻咽喉头颈外科接受开放性手术治疗的喉癌患者80例,随机分为实验组和对照组,每组各40例。实验组患者采用人工鼻后置的改良法连接气管切开后通气管路,对照组则使用传统方法进行连接。观察两组患者管路连接时间、术中血氧水平、胸前皮肤压伤情况与脱管率,比较分析改良通气管路连接法在喉癌手术中的应用价值。结果两组管路连接时间、术中血氧水平差异比较异无显著性统计学意义(P>0.05);实验组患者胸前皮肤压力性损伤发生率及术中脱管率均显著低于对照组(P<0.05)。结论喉癌手术患者气管切开后应用改良法连接通气管路,可有效降低胸前壁压伤及术中脱管发生率。Objective To investigate the clinical value of modified airway connecting method with ventilation tube following tracheotomy in laryngeal carcinoma surgery.Methods Included in this trial were 80 patients diagnosed with laryngeal cancer and treated with open surgery in our Hospital from Jan.2018 to Oct.2020.They were randomly divided into experimental group(EG)and control group(CG),with 40 patients in each.The modified method,with the artificial nose placed posteriorly,was used to connect the airway line following tracheotomy for the cases in EG,while the traditional method was used to connect the airway line following tracheotomy in those of CG.Then,determined were such indicators as the connection time with airway line,blood oxygen level during the operation,the injuring status of skin in the front of the chest,and tube detachment rate of endotracheal tube during the operation to evaluate the applying value of the modified airway connecting method in this kind of surgery in a comparative way between the two groups.Result While there were no significantly statistical significance in pipeline connection time and blood oxygen level during the operation between the two groups(P>0.05)as evaluated as above,the incidence rate of pressure injury on the skin in the front of chest and the tube detachment rate of endotracheal tube during the operation in EG were obviously lower than that in CG(P<0.05).Conclusion The use of modified airway connecting method holds significantly clinical applying value for cases receiving laryngeal carcinoma surgery with ventilation tube used following tracheotomy to effectively reduce the occurrence of skin compression injury in the front chest wall and he tube detachment rate of endotracheal tube during the operation.
