日间手术全麻拔除多颗智齿的临床应用及评价

摘　　要：目的分析新冠疫情常态化期间我院日间手术全麻下拔除多颗智齿的病历资料,总结疫情常态化期间及之后开展日间手术全麻拔除多颗智齿手术的诊疗规范性、可行性和必要性。方法收集我院2020年6月~2021年3月间日间手术全麻下拔除多颗智齿的病历,统计并总结手术的一般情况、术前准备、选择该方法的原因、拔除牙位、离院状况、复诊随访评价、诊疗心理及满意度评估。分析该方式术前、术中、术后特点。结果共收集282个病例,男138例,女144例,平均年龄(28.05±7.25)岁。疫情常态化期间,日间手术全麻拔牙的术前检查及新型冠状病毒肺炎核酸检测必不可少。选择全麻的原因包括:对手术恐惧或追求术中舒适,希望缩短疗程,担心术后恢复不佳,对局麻药物过敏或效果不佳,牙科就诊晕厥史,咽部敏感,牙齿位置特殊等。拔除的牙位主要是三颗及以上的智齿。所有病例均手术顺利并且安全离院,术后牙科畏惧值降低,诊疗舒适度及满意度较高。所有患者均未出现院内感染。结论日间手术拔除多颗智齿为口腔科拔牙手术的新的诊疗途径。在医院规范管理、手术室合理运作、各学科医护人员互相配合的条件下,能给患者带来诸多便利,该方式值得推广。Objective To evaluate the standardization,feasibility and necessity of teeth extraction under general anesthesia in day surgery during and after the COVID-19 pandemic.Methods The patients accepting teeth extraction under general anesthesia in day surgery were included in this study from June 2020 to March 2021.The following information was recorded and summed,including the general situation of operation,preoperative preparation,the reasons for choosing this therapy,the position of teeth removed,status when leaving hospital,psychology and satisfaction evaluation.Results A total of 282 cases were collected in this study.There were 128males and 144 females,and the average age was(28.05±7.25).It is essential to take pre-surgery examination and COVID-19 nucleic acid testing before day surgery.The reasons for choosing general anesthesia including:being afraid of surgery or pursuing of operation comfort,wanting to shorten the course of treatment,worrying about the poor postoperative recovery,being allergenic or having poor response to local anesthesia drug,having history of dental syncope,having throat sensitivity,or having special teeth position,etc.The number of teeth extracted of all cases is three or more.All cases were successfully operated and safely discharged from the hospital.After the operation,the dental fear value is reduced,and the comfort as well as the satisfaction of diagnosis and treatment is better.None of the patients developed nosocomial infection.Conclusion Extraction of multiple wisdom teeth under general anesthesia in day surgery can be a new way of treatment for dental extraction.Under the condition of standardized hospital management,reasonable operation of operating room and mutual cooperation of medical staff of various disciplines,it can bring a lot of convenience to patients,and this method is worth promoting.
