摘 要:目的探究Revolution CT行上腹部增强扫描在肝局灶性增生(FNH)与肝细胞癌(HCC)鉴别中的应用。方法回顾分析2021年1月~2022年9月就诊于本院的肝占位性病变患者87例的临床资料,依据患者病理结果将其分为FNH组(FNH患者,n=47)和HCC组(HCC患者,n=40),对比两组门脉期占位区CT值/肝组织CT值(LHRHU)、占位区碘浓度/肝组织碘浓度(LHRIC)、占位区灰度平均值,做ROC曲线分析上述指标鉴别价值,并用Logistic分析上述指标与肝脏占位病变良恶性的相关性。结果HCC组相较于FNH组,其LHRIC、LHRHU、占位区灰度平均值更低,差异有统计学意义(P<0.05),上述指标鉴别FNH和HCC的价值良好。对应的cut~off值分别为≤0.97、≤1.31、≤101.33。结论临床可将Revolution 256排CT上腹部增强扫描应用于肝脏占位性病变的筛查中,以早期诊断HCC,改善患者预后。Objective To explore the application of upper abdominal enhanced scan with Revolution CT in the differential diagnosis of hepatic focal nodular hyperplasia(FNH)and hepatocellular carcinoma(HCC).Methods The clinical data of 87 patients with liver space~occupying lesions who were treated in the hospital were retrospectively analyzed between January 2021 and September 2022.According to the pathological results,the patients were divided into FNH group(patients with FNH,n=47)and HCC group(patients with HCC,n=40).The lesion~to~liver ratio of CT value(LHRHU),lesion~to~liver ratio of iodine concentration(LHRIC)and average gray value of occupying area in the portal phase were compared between the two groups.ROC curve was used to analyze the differential value of the above indicators,and Logistic analysis was applied to analyze the correlation between the above indicators and benign and malignant liver space~occupying lesions.Results The LHRIC,LHRHU and average gray value of occupying area in HCC group were lower compared with those in FNH group(P<0.05).The above indicators had good value on differentiating FNH from HCC,and their corresponding cut~off values were≤0.97,≤1.31 and≤101.33 respectively.Conclusion Revolution 256~slice CT for upper abdominal enhanced scan can be used in the screening of liver space~occupying lesions for the early diagnosis of HCC and improvement of prognosis of patients.
