肺部CTA多期血管融合在肺部占位病变鉴别诊断中的应用

摘  要:目的探讨肺部CT血管造影检查(CTA)和多期血管图像融合技术在肺部占位病变鉴别诊断中的应用。方法回顾性分析2020年7月~2022年6月于本院行手术治疗的肺部占位病变患者97例,所有患者均行多排螺旋肺部CT平扫与静脉期及动脉期增强扫描。将静脉期与动脉期血管进行三维重建,并对图像进行融合。以术后病理学检查结果为“金标准”,比较肺部CTA动脉期图像重建与肺部CTA联合多期血管图像融合技术对肺部占位性病变良恶性的准确性差异。结果经术后病理学检查97例患者中良性病变28例(28.87%),恶性病变69例(71.13%);CTA动脉期检查诊断肺部占位病变的准确度为89.69%(87/97),Kappa值为0.759,CTA多期图像融合检查诊断肺部占位病变的准确度为97.94%(95/97),Kappa值为0.951,准确度明显高于CTA动脉期检查(χ^(2)=5.685,P=0.017)。结论肺部CTA和多期血管图像融合技术能直观显示肺部占位病变血管组成,并有效鉴别良恶性病变。Objective To explore e the application of lung CT angiography(CTA)and multi~phase vascular image fusion technology in the differential diagnosis of lung space~occupying lesions.Methods 97 patients with lung space~occupying lesions who underwent surgical treatment in the hospital were retrospectively analyzed between July 2020 and June 2022.All patients underwent multi~slice spiral lung CT plain scan and venous phase and arterial phase enhanced scans.Three~dimensional reconstruction of venous and arterial phase vessels was performed and the images were fused.The postoperative pathological examination was taken as the"gold standard"to compare the differences in accuracy of lung CTA arterial phase image reconstruction and lung CTA combined with multi-phase vascular image fusion technology on benign and malignant lung space~occupying lesions.Results Among 97 patients with postoperative pathological examination,there were 28 cases(28.87%)of benign lesions and 69 cases(71.13%)of malignant lesions.The accuracy and Kappa value in the diagnosis of lung space~occupying lesions were 89.69%(87/97)and 0.759 of CTA arterial phase examination and were 97.94%(95/97)and 0.951 of CTA multi~phase image fusion examination.The accuracy of CTA multi~phase image fusion examination was significantly higher than that of CTA arterial phase examination(χ^(2)=5.685,P=0.017).Conclusion Lung CTA and multi~phase vascular image fusion technology can visually show the vascular compositions of lung space~occupying lesions and effectively distinguish benign and malignant lesions.
