血清lncRNA MIAT表达水平与动脉瘤性蛛网膜下腔出血严重程度及预后的关系

摘　　要：目的研究动脉瘤性蛛网膜下腔出血(aSAH)患者血清长链非编码RNA心肌梗死相关转录本(lncRNA MIAT)表达水平与疾病严重程度及预后的关系。方法纳入2019年1月至2021年7月该院收治的117例aSAH患者作为aSAH组,收集aSAH患者的临床资料,根据Hunt-Hess分级法对患者aSAH严重程度进行分级;依据格拉斯哥预后量表(GOS)评分评估患者预后。纳入同期该院健康体检者109例作为对照组。采用实时荧光定量PCR检测血清lncRNA MIAT表达水平。绘制受试者工作特征(ROC)曲线分析血清lncRNA MIAT表达水平对aSAH患者预后不良的预测价值。采用多因素Logistic回归分析aSAH患者预后不良的影响因素。结果aSAH组血清lncRNA MIAT表达水平明显高于对照组,差异有统计学意义(P<0.05)。根据Hunt-Hess分级法将患者分为Ⅰ~Ⅱ级62例,Ⅲ级34例,Ⅳ级21例。Ⅳ级aSAH患者血清lncRNA MIAT表达水平明显高于Ⅲ级和Ⅰ~Ⅱ级患者,Ⅲ级患者血清lncRNA MIAT表达水平明显高于Ⅰ~Ⅱ级患者,差异有统计学意义(P<0.05)。ROC曲线分析结果显示,血清lncRNA MIAT预测aSAH患者预后不良的曲线下面积为0.848(95%CI:0.773~0.923),截断值为2.42,灵敏度为78.0%,特异度为71.1%。多因素Logistic回归分析结果显示,lncRNA MIAT表达水平、Hunt-Hess分级是aSAH患者预后不良的独立影响因素(P<0.05)。结论血清lncRNA MIAT表达水平是aSAH患者预后不良的影响因素,可用于评估患者疾病严重程度及预后。Objective To study the relationship between the expression level of serum long-chain non-coding RNA myocardial infarction associated transcript(lncRNA MIAT)and the severity and prognosis of the disease in patients with aneurysmal subarachnoid hemorrhage(aSAH).Methods A total of 117 patients with aSAH admitted to the hospital from January 2019 to July 2021 were included in the aSAH group.Clinical data of aSAH patients were collected,and the severity of aSAH was graded according to Hunt-Hess grading method.Glasgow Out come Scale(GOS)score was used to evaluate the prognosis of patients.A total of 109 healthy people underwent physical examination in the same period were included as the control group.The expression level of serum lncRNA MIAT was detected by real-time quantitative PCR.Receiver operating characteristic(ROC)curve was plotted to analyze the predictive value of serum lncRNA MIAT expression level for poor prognosis in aSAH patients.Multivariate Logistic regression was used to analyze the influencing factors of poor prognosis in aSAH patients.Results Serum lncRNA MIAT expression level in aSAH group was significantly higher than that in control group,and the difference was statistically significant(P<0.05).According to Hunt-Hess grading,the patients were divided into 62 cases of gradeⅠ-Ⅱ,34 cases of gradeⅢ,and 21 cases of gradeⅣ.The serum expression level of lncRNA MIAT in gradeⅣaSAH patients was significantly higher than that in gradeⅢand gradeⅠ-Ⅱpatients,and the serum expression level of lncRNA MIAT in gradeⅢpatients was significantly higher than that in gradeⅠ-Ⅱpatients,the differences were statistically significant(P<0.05).ROC curve analysis results showed that the area under the curve of serum lncRNA MIAT in predicting poor prognosis of aSAH patients was 0.848(95%CI:0.773-0.923),the cut-off value was 2.42,the sensitivity was 78.0%and the specificity was 71.1%.Multivariate Logistic regression analysis showed that the expression level of lncRNA MIAT and Hunt-Hess grading were independ
