经肛门微创手术与传统经肛局部切除术治疗直肠肿瘤患者的对比研究

摘　　要：目的 探讨经肛门微创手术(TAMIS)与传统经肛局部切除手术(TAE)治疗直肠肿瘤的临床效果。方法 前瞻性纳入2019-06—2021-06于南阳市中心医院普通外科手术的直肠肿瘤患者,依据不同手术方式分为TAMIS组(32例)和TAE组(27例)。比较2组患者的基线资料、手术时间、术中出血量、术后数字评分法(NRS)评分、住院时间、并发症发生率,以及术后随访3个月期间的复发率。结果 共纳入59例患者,2组患者的基线资料、切缘阴性率、并发症发生率和复发率差异均无统计学意义(P>0.05)。TAMIS组患者的手术时间、术中出血量、术后第1天的NRS评分和住院时间均短(小)于TAE组,差异均有统计学意义(P<0.05)。结论 TAMIS与TAE治疗直肠肿瘤的切缘阴性率、并发症发生率和复发率差异均无统计学意义。但TAMIS具有手术时间短、术中出血量少、术后疼痛轻、并发症少、住院时间短等优势。Objective To investigate the clinical effect of transanal minimally invasive surgery(TAMIS) and traditional transanal regional anal resection(TAE) in the treatment of rectal tumors. Methods The rectal tumor patients from 2019-06 to 2021-06 were prospectively included and were divided into TAMIS group(32 patients) and TAE group(27 patients) according to different surgical methods. Baseline data, operation time, intraoperative blood loss, postoperative numerical score(NRS) score, mean length of stay, complication rate, and recurrence rate during the 3-month postoperative follow-up were compared in 2 groups. Results A total of 59 patients were included, and there were no significant differences in the baseline data, negative margin rate, complication rate, and recurrence rate of 2 group patients(P>0.05).The operation time, intraoperative blood loss, NRS score and hospital stay length of the patients in the TAMIS group were shorter(smaller) than those of TAE group. The differences were all statistically significant(P<0.05). Conclusion There was no significant difference in negative margin rate, complication rate and recurrence rate between TAMIS and TAE. However, TAMIS has the advantages of short operation time, less intraoperative bleeding, less postoperative pain, less complications and short hospital stay.
