摘 要:作为临床中常见的恶性肿瘤之一,甲状腺癌在内分泌系统肿瘤中具有较高的发病率。甲状腺癌可发生于各个年龄阶段,主要好发于青壮年,平均发病年龄为40岁,许多证据表明年龄是甲状腺癌的危险因素之一,与甲状腺癌的发病呈正相关。激素、生殖因素是女性患者的危险因素,对于男性而言,甲状腺肿或甲状腺结节史、恶性肿瘤家族史是高危因素,这些因素同样适用于女性群体,潜在风险的增加使女性患者数量居高不下。甲状腺癌病理类型分为乳头状癌、滤泡状癌、髓样癌和未分化癌,其中乳头状癌和滤泡状癌统称为分化型甲状腺癌,占总体比例90%以上,五年生存率可达98%左右,及时发现、及时治疗,预后相对较好。髓样癌和未分化癌在临床所占的比例较少,恶性程度较高、预后相对较差。医疗人员应积极做好临床诊疗工作,从而有效提升对于该病的诊断水平,为患者及时得到确诊与治疗奠定坚实的基础与保障。就目前而言,临床一般多采用CT检查对该病患者进行诊断,其中,多排螺旋CT的平扫配合多排螺旋灌注成像可以有效实现对于患者情况的合理分析,其影像资料可以为临床诊断提供丰富的依据。本文对多排螺旋CT诊断甲状腺癌研究进展进行综述,以期为提高甲状腺癌临床诊断水平提供帮助。As one of the most common malignant tumors in clinic,thyroid carcinoma has high incidence rate in endocrine system tumors. Thyroid carcinoma can occur at all ages,mainly in young adults. The average age of onset is 40 years old. Many evidences show that age is one of the risk factors for thyroid carcinoma,and it is positively correlated with the incidence of thyroid carcinoma. Hormones and reproductive factors are risk factors for female patients. For males,the history of goiter or thyroid nodules and family history of malignant tumors are high-risk factors. These factors also apply to female groups. The increase in potential risk keeps the number of female patients at a high level. Pathological types of thyroid carcinoma are divided into papillary carcinoma, follicular carcinoma, medullary carcinoma and undifferentiated carcinoma,among which papillary carcinoma and follicular carcinoma are collectively referred to as differentiated thyroid carcinoma,accounting for more than 90% of the total,and the five-year survival rate can reach about 98%. If detected and treated in time,the prognosis is relatively good.Medullary carcinoma and undifferentiated carcinoma account for a small proportion in the clinic,with a high degree of malignancy and a relatively poor prognosis. Medical staff should actively do a good job in the clinical diagnosis and treatment,so as to effectively improve the diagnosis level of the disease,and lay a solid foundation and guarantee for the timely diagnosis and treatment of patients. At present,CT examination is commonly used in clinical diagnosis of the disease. The multi-slice spiral CT combined with multi-slice spiral perfusion imaging can effectively realize the reasonable analysis of the patient’s situation,and its image data can provide rich basis for clinical diagnosis. This article reviews the research progress in the diagnosis of thyroid carcinoma with multi-slice spiral CT,in order to improve the clinical diagnosis of thyroid carcinoma.
