摘 要:目的:观察有氧运动对心力衰竭(心衰)大鼠心脏交感神经功能的影响,并探讨心肌去甲肾上腺素转运蛋白(NET)在其间的可能作用机制。方法:38只Wistar大鼠随机分为假手术组(Sham)、心衰安静组(HF-sed)和心衰运动组(HF-ex)。Sham和HF-sed组大鼠在鼠笼内安静饲养,HF-ex组进行10周中等强度跑台运动。实验结束后,利用超声心动图检测心脏结构与功能,心率变异性(HRV)频域分析获取自主神经功能,行Masson染色观察心脏组织病理学变化,高压液相色谱法测定血浆和心肌去甲肾上腺素(NE)水平,实时荧光定量PCR和Western blot检测心脏交感神经节NET mRNA和蛋白,心肌总NET、膜NET和胞浆NET蛋白表达量。结果:与Sham组比较,HF-sed组发生病理性心脏肥大,心肌纤维化,交感神经活性增加(P<0.05),心肌和血浆NE升高(P<0.05),心功能下降(P<0.05);分子生物学测定发现,心脏交感神经节NET mRNA表达下调(P<0.05),心肌总NET和膜NET蛋白下降(P<0.05),胞浆NET升高(P<0.05)。与HF-sed组比较,HF-ex组心脏由病理性肥大向生理性肥大转变,心肌纤维化减轻,交感神经活性降低(P<0.05),心肌和血浆NE含量下降(P<0.05),心功能增强(P<0.05);心脏交感神经节NET mRNA表达上调(P<0.05),心肌总NET和膜NET蛋白升高(P<0.05),胞浆NET下降(P<0.05)。结论:有氧运动通过上调NET表达量并抑制其内化,进而改善心衰大鼠交感神经过度兴奋状态并延缓心脏重塑和心衰进程。Objective:To explore the effects of aerobic exercise on cardiac sympathetic nerve function and investigate the possible role of norepinephrine transporter(NET)in rats of heart failure.Methods:Thirty-eight Wistar rats were randomly divided into sham operation(Sham),heart failure sedentary(HF-sed)or heart failure exercise(HF-ex)groups.Rats of Sham and HF-sed groups kept sedentary while those of HF-ex group performed moderate intensity treadmill running exercise intervention for 10 weeks.After experiments,cardiac structure and function by ultrasoundcardiogram,cardiac histopathological change by Masson staining,plasma and myocardial norepinephrine(NE)level by high pressure liquid chromatography,autonomic nervous function by frequency domain analysis of heart rate variability(HRV),cardiac sympathetic ganglion NET mRNA and protein,and myocardial total,membranous and cytoplasm NET protein expression by real-time Q-PCR and Western blot were measured.Results:Compared with Sham group,rats in HF-sed group presented pathological cardiac hypertrophy,myocardial fibrosis,sympathetic activity elevation(P<0.05),myocardial and plasma NE increased(P<0.05),cardiac function reduced(P<0.05);molecular biology assay discovered that cardiac sympathetic ganglion NET mRNA downregulated(P<0.05),myocardial total and membranous NET protein reduced(P<0.05)while cytoplasm NET protein upregulated(P<0.05).Compared with HF-sed group,animals in HF-ex group showed conversion from pathological to physiological cardiac hypertrophy,reduced myocardial fibrosis,descended sympathetic activity(P<0.05)and myocardial and plasma NE(P<0.05),cardiac function enhanced(P<0.05);cardiac sympathetic ganglion NET mRNA upregulated(P<0.05),myocardial total and membranous NET protein increased(P<0.05)while cytoplasm NET protein downregulated(P<0.05).Conclusion:Aerobic exercise improved sympathetic overexcitation and delayed cardiac remodeling and heart failure by upregulating norepinephrine transporter gene expression and inhibiting its internalization.
