摘 要:目的探讨脑小血管病(CSVD)患者外周血G蛋白耦联受体(GPR30)mRNA、血小板与淋巴细胞比值(PLR)、神经元PAS结构域蛋白4(NPAS4)表达及与认知功能、短期预后的关系。方法选取2019年1月—2021年2月收治的CSVD 200例,依据是否发生认知功能障碍分为发生组85例与未发生组115例。比较2组临床资料、外周血GPR30 mRNA、PLR、NPAS4表达水平,分析外周血GPR30 mRNA、PLR、NPAS4表达与CSVD患者认知功能障碍的关系,评价其对CSVD患者认知功能障碍的诊断价值及与CSVD发生认知功能障碍患者短期预后不良的关系。结果2组文化程度、腔隙性脑梗死病灶数目、脑白质病变及脑血管病面积比较差异有统计学意义(P<0.01);发生组外周血GPR30 mRNA水平低于未发生组,PLR、NPAS4表达水平高于未发生组(P<0.01);外周血GPR30 mRNA、PLR、NPAS4表达与CSVD患者认知功能障碍显著相关(P<0.01);外周血GPR30 mRNA、PLR、NPAS4表达联合诊断曲线下面积最大;CSVD发生认知功能障碍患者外周血GPR30 mRNA、PLR、NPAS4高表达时,短期预后不良发生风险是低表达的0.274、3.949、3.276倍。结论CSVD患者外周血GPR30 mRNA表达水平下调,PLR、NPAS4表达水平上调,且其表达与认知功能障碍及短期预后有关,可用于CSVD患者认知功能障碍诊断及短期预后预测。Objective To investigate the expression of peripheral blood G protein-coupled receptor(GPR30)mRNA,platelet-to-lymphocyte ratio(PLR),neuronal PAS structural domain protein 4(NPAS4)and their relationship with cognitive function and short-term prognosis in patients with cerebral small vessel disease(CSVD).Methods Two hundred patients with CSVD treated from January 2019 to February 2021 were selected and grouped into the occurrence group(n=85)and non-occurrence group(n=115)based on occurrence of cognitive dysfunction.The clinical data,peripheral blood GPR30 mRNA,PLR,and NPAS4 expression levels between the two groups were compared,and the relationship between peripheral blood GPR30 mRNA,PLR,and NPAS4 expression and cognitive dysfunction in CSVD patients was analyzed,to evaluate their diagnostic value in cognitive dysfunction in CSVD patients and their relationship with poor short-term prognosis in CSVD patients with cognitive dysfunction.Results The level of education,the number of lacunar infarction(LI)lesions,white matter lesions(WMLs)and cerebrovascular disease area were significantly different between the two groups(P<0.01).The levels of GPR30 mRNA in peripheral blood of the occurrence group were lower than those of the non-occurrence group,and the expression levels of PLR and NPAS4 were higher than those of the non-occurrence group(P<0.01).The expressions of GPR30 mRNA,PLR and NPAS4 in peripheral blood were significantly correlated with cognitive dysfunction in CSVD patients(P<0.05).The area under the receiver operating characteristic(ROC)curve(AUC)of GPR30 mRNA,PLR and NPAS4 expression in peripheral blood was the largest.When GPR30 mRNA,PLR and NPAS4 were highly expressed in peripheral blood of CSVD patients with cognitive dysfunction,the risk of poor short-term prognosis was 0.274,3.949,3.276 times higher than that of low expression.Conclusion The expression of GPR30 mRNA was significantly down-regulated,while the expression of PLR and NPAS4 were up-regulated in peripheral blood of CSVD patients,and their expre
