摘 要:目的 系统评价中国胰腺癌患者肠道菌群多样性组成谱。方法 计算机检索The Cochrane Library、PubMed、Web of Science、EMbase、CNKI、CBM、WanFang Data和VIP数据库,搜集中国胰腺癌患者肠道菌群多样性组成谱的横断面研究,检索时限均从建库至2021年12月31日。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用RevMan 5.3软件进行Meta分析。结果 最终纳入7个横断面研究,包括250例胰腺癌患者,166例健康对照人群。Meta分析结果显示,与健康对照人群相比,胰腺癌患者肠道菌群α多样性Shannon指数降低。高通量测序结果显示,变形菌门、普氏菌属在胰腺癌患者中相对丰度高于健康者;厚壁菌门、栖粪杆菌属、双歧杆菌属和梭菌属在胰腺癌患者中相对丰度低于健康者。结论 现有证据表明胰腺癌患者肠道菌群有一定特性,变形菌门、普氏菌属在胰腺癌患者中相对丰度较高;厚壁菌门、栖粪杆菌属、双歧杆菌属和梭菌属在胰腺癌患者中相对丰度较低。受纳入研究数量和质量的限制,上述结论尚待更多高质量研究予以验证。Objective To systematically review the intestinal flora diversity profile of pancreatic cancer patients.Methods The Cochrane Library, PubMed, Web of Science, EMbase, CNKI, CBM, WanFang Data and VIP databases were electronically searched to collect cross-sectional studies on the intestinal flora diversity profile of pancreatic cancer patients from inception to December 31, 2021. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies;then, meta-analysis was performed by using RevMan 5.3 software. Results A total of 7 cross-sectional studies involving 250 pancreatic cancer patients and 166 healthy controls were included. The results of meta-analysis showed that: compared with the healthy control group, the intestinal flora of patients with pancreatic cancer α reduced diversity with the Shannon index. High-throughput sequencing found that Proteobacteria and Prevotella were more abundant in pancreatic cancer patients, Firmicutes, Faecalbacterium, Bifidobacterium and Clostridium in pancreatic cancer patients was lower. Conclusion Current evidence shows that the intestinal flora of pancreatic cancer patients has certain characteristics. Proteobacteria and Prevotella are relatively abundant in pancreatic cancer patients. Due to limited quality and quantity of the included studies, more high-quality studies are needed to verify above conclusion.
