半夏白术天麻汤及针刺疗法辅助治疗对痰湿瘀阻型缺血性脑卒中急性期患者的疗效观察

摘　　要：目的:分析半夏白术天麻汤及针刺疗法辅助治疗痰湿瘀阻型缺血性脑卒中(Ischemic stroke,IS)急性期患者的临床疗效。方法:纳入郑州颐和医院2019年10月至2021年2月收治的痰湿瘀阻型IS急性期患者134例。按照临床治疗方案不同,分为西药组(67例)、中西医联合组(67例)。在常规对症支持治疗的基础上,西药组采用依达拉奉治疗,中西医联合组在西药组治疗基础上采用半夏白术天麻汤+健脾调神针针刺疗法辅助治疗。治疗2 w后,评估两组的疗效和中医证候积分;采用美国国立卫生研究院卒中评分量表评估两组患者的神经功能,采用脑卒中专用生活质量评分量表评估患者的生活质量,并比较治疗前后凝血功能和血流动力学指标。结果:中西医联合组治疗总有效率(89.55%)高于西药组(76.12%)(P<0.05);与治疗前相比,治疗2 w后,两组的中医证候积分、卒中评分均明显降低,生活质量评分明显升高(P<0.05),纤维蛋白原明显降低,凝血酶原时间(Prothrombin time,PT)、活化部分的凝血活酶时间(Activated partial thromboplastin time,APTT)明显变长(P<0.05),峰值流速、平均流速均明显升高(P<0.05),其中中西医联合组各项指标改善更为显著(P<0.05)。结论:痰湿瘀阻型IS急性期患者,在西药依达拉奉治疗的基础上,给予半夏白术天麻汤及针刺疗法辅助治疗的效果显著,能改善凝血功能、改善血流动力学,减轻神经功能损伤,提高生活质量。Objective:To explore the clinical efficacy of Banxia Baizhu Tianma decoction and acupuncture in the treatment of acute ischemic stroke(IS)of phlegm dampness and blood stasis type.Methods:134 IS patients with acute stage of phlegm dampness and blood stasis type in Zhengzhou Yihe Hospital from October 2019 to February 2021 were included.According to different clinical treatment schemes,they were divided into western medicine group(67 cases)and combined treatment group(67 cases).On the basis of routine symptomatic support treatment,western medicine group was given edaravone treatment,and the combined treatment group was given Banxia Baizhu Tianma decoction+Jianpi tiaosheng needle acupuncture treatment on the basis of western medicine group.After 2 weeks of treatment,the curative effects and TCM syndrome scores of the two groups were evaluated.The neurological function was evaluated by National Institutes of Health Stroke Scale,and the quality of life of the patients was evaluated by Strokespecific Quality of Life Scale.The coagulation function and hemodynamic indexes were compared were compared beween the two groups.Results:After 2 weeks of treatment,the total effective rate(89.55%)in the combination group was higher than that in the western medicine group(76.12%)(P<0.05);compared with before treatment,after 2 weeks of treatment,TCM syndrome score and stroke score were significantly reduced,life quality score was significantly increased(P<0.05),fibrinogen was significantly reduced,Prothrombin time(PT)and activated partial thromboplastin time,APTT)were significantly longer(P<0.05),the peak flow rate and average flow rate were significantly increased(P<0.05)in two groups,especially in the combined treatment group(P<0.05).Conclusion:On the basis of edaravone,the adjuvant therapy of Banxia Baizhu Tianma decoction and acupuncture can significantly improve coagulation function,improve hemodynamics,reduce neurological damage and improve quality of life in patients with acute is of phlegm dampness and blood stasis type.
