激光周边虹膜切除术治疗原发性房角关闭的临床效果

摘　　要：目的探讨激光周边虹膜切除术在治疗原发性房角关闭中的疗效。方法前瞻性选取2020年1月至2021年12月于首都医科大学附属北京友谊医院眼科经激光周边虹膜切除术治疗21例42眼原发性房角关闭患者,其中6例伴眼压升高,均无视神经损伤。在局部麻醉下以YAG激光进行周边虹膜打孔,平均能量8 mJ。于术前,术后1 h、1 d、1周及1个月监测眼压、最佳矫正视力(BCVA)和前房深度的变化。结果术后1 d、1周和1个月眼压为(19.80±1.23)、(18.20±1.55)、(18.45±2.21)mmHg,较术前眼压[(25.80±7.45)mmHg]有明显降低,差异均有统计学意义(P<0.05)。观察患者治疗前和治疗后1 d、1周及1个月的BCVA比较,差异均无统计学意义(P>0.05)。术后1周和术后1个月前房深度为(2.26±0.09)、(2.28±0.08)、(2.27±0.11)mm,较术前[(2.12±0.12)mm]明显加深,差异均有统计学意义(P<0.05)。结论LPI在临床治疗原发性房角关闭患者中疗效确切,值得进行推广和应用。Objective To explore the effect of laser peripheral iridectomy in the treatment of primary angle closure.Methods A total of 42 eyes of 21 patients with primary atrial angle closure after laser peripheral iridectomy were prospectively selected from the Department of Ophthalmology,Beijing Friendship Hospital,Capital Medical University From January 2020 to December 2021,of which 6 cases were accompanied by increased intraocular pressure,without optic nerve damage.Peripheral iris perforation with YAG laser under local anesthesia,with an average energy of 8 mJ.The changes of intraocular pressure,best corrected visual acuity(BCVA)and anterior chamber depth were monitored before operation,1 hour after operation,1 day after operation,1 week and 1 month after operation.Results The intraocular pressure was(19.80±1.23),(18.20±1.55),(18.45±2.21)mmHg at 1 day,1 week and 1 month after operation,which was significantly lower than that before operation[(25.80±7.45)mmHg],the difference was statistically significant(P<0.05).There was no significant difference in BCVA before treatment and 1 day,1 week and 1 month after treatment(P>0.05).The anterior chamber depth at 1 day,1 week and 1 month after the operation were(2.26±0.09),(2.28±0.08)and(2.27±0.11)mm,which was significantly deeper than before operation[(2.12±0.12)mm],the difference was statistically significant(P<0.05).Conclusion LPI has a definite effect and is worthy of promotion and application in the clinical treatment of patients with primary angle closure.
