两种材料用于乳磨牙深龋意外露髓活髓切断术中的临床效果观察

摘　　要：目的通过应用生物陶瓷根管修复材料(即iRootBPPlus)与氢氧化钙糊剂来观察活髓切断术在乳牙深龋意外露髓的临床效果。方法选择2020年6月至2022年1月在连云港市中医院口腔科接受乳磨牙深龋意外露髓治疗的患者68例作为研究对象并随机分组,其中34例患者使用生物陶瓷根管修复材料开展活髓切断术治疗(观察组),另外34例患者应用氢氧化钙糊剂活髓切断术治疗(对照组),比较分析两组患者在接受诊治后1.5、3、6个月的不良反应等指标。结果观察组患者1.5、3、6个月盖髓成功率为97.06%、97.06%、94.12%,高于对照组的88.24%、83.35%、73.53%,差异有统计学意义(P<0.05);观察组出现1例术后不适、1例牙龈瘘管的并发症,在改做根管治疗后得到彻底缓解,观察组患者的并发症总发生率为5.88%,低于对照组的20.59%,差异有统计学意义(P<0.05);观察组总满意率为94.11%,高于对照组的73.52%差异有统计学意义(P<0.05)。结论应用生物陶瓷根管修复材料活髓切断术在乳牙深龋意外露髓的临床效果显著,具有较强的可靠性与安全性,临床推广应用价值较高。Objective To observe the clinical effect of vital pulpotomy on accidental pulp exposure of deciduous teeth with deep caries by applying bioceramic root canal repair material(iRootBPPlus)and calcium hydroxide paste.Methods A total of 68 patients who received accidental pulp exposure treatment of deciduous molar with deep caries in the Department of Stomatology,Lianyungang Hospital of Traditional Chinese Medicine from June 2020 to January 2022 were selected as the study samples and randomly divided into two groups.34 patients underwent vital pulpotomy with bioceramic root canal repair material(the observation group),while the other 34 patients underwent vital pulpotomy with calcium hydroxide paste(the control group).The adverse reactions and other data of the two groups in 1.5 months,3 months and 6 months after the treatment were compared.Results The success rates of pulp capping in the observation group at 1.5 months,3 months and 6 months(97.06%,97.06%and 94.12%)were higher than those in the control group(88.24%,83.35%and 73.53%),with statistically significant differences(P<0.05).One case of postoperative discomfort and one case of complication of gingival fistula in the observation group were completely relieved after being converted to root canal treatment.Therefore,the total incidence of complications in the observation group was 5.88%,which was lower than that in the control group(20.59%),with statistically significant difference(P<0.05).The total satisfaction rate in the observation group(94.11%)was higher than that in the control group(73.52%),with statistically significant difference(P<0.05).Conclusion Vital pulpotomy with bioceramic root canal repair material has obvious clinical effect on accidental pulp exposure of deciduous molar with deep caries,with strong reliability and safety,it has high value of clinical promotion and application.
