摘 要:目的 分析非酒精性脂肪性肝病(NAFLD)与急性胰腺炎(AP)严重程度的相关性。方法 收集我院AP住院患者331例的临床资料,按照严重程度分为轻度急性胰腺炎(MAP)、中度重症急性胰腺炎(MSAP)、重度急性胰腺炎(SAP),进行AP严重程度影响因素的多元logistic回归分析;将患者分为非NAFLD组与NAFLD组,比较有无NAFLD与AP严重程度的关系;剔除掉非NAFLD组,将NAFLD组按照严重程度分为轻度NAFLD、中度NAFLD、重度NAFLD,对NAFLD严重程度与AP严重程度进行卡方线性趋势检验。结果 NAFLD是影响AP严重程度的独立危险因素(r=1.177,P<0.05);AP病情随着NAFLD严重程度而加重(χ^(2)=8.605,P<0.05)。结论 非酒精性脂肪性肝病是影响急性胰腺炎严重程度的独立危险因素,且非酒精性脂肪性肝病的严重程度与急性胰腺炎的病情严重程度呈正相关。Objective To explore the correlation between nonalcoholic fatty liver disease(NAFLD) and the severity of acute pancreatitis(AP).Methods The clinical datas of 331 AP inpatients in our hospital were collected and divided into mild acute pancreatitis(MAP), moderate severe acute pancreatitis(MSAP) and severe acute pancreatitis(SAP) according to the severity.Multiple logistic regression analysis was performed on the influencing factors of AP severity.Then the patients were divided into non-NAFLD group and NAFLD group to compare the relationship between NAFLD and AP severity.Finally, the non-NAFLD group was excluded, and the NAFLD group was divided into mild NAFLD, moderate NAFLD and severe NAFLD according to the severity.The Chi-square linear trend test was conducted for the severity of NAFLD and AP.Results NAFLD was an independent risk factor affecting the severity of AP(r=1.177,P<0.05).AP condition aggravated with the severity of NAFLD(χ^(2)=8.605,P<0.05).Conclusion Nonalcoholic fatty liver disease is an independent risk factor affecting the severity of acute pancreatitis, and the severity of acute pancreatitis is strongly associated with the severity of nonalcoholic fatty liver disease.
