基于NIHSS评分、血液流变学及内皮功能评价扎冲十三味丸治疗脑梗死后偏瘫的疗效

摘　　要：目的探讨扎冲十三味丸对脑梗死后偏瘫患者功能缺损评分(NIHSS)、上肢运动功能的影响。方法采用随机数字表法将2019年2月-2022年2月收治的150例脑梗死后偏瘫患者分为A组和B组,各75例。2组均给予常规治疗及长春西汀片,B组此基础上给予扎冲十三味丸。2组均连续治疗1个月。结果治疗1个月后,B组总有效率为92.00%,高于A组的73.00%(P<0.05)。治疗1个月后,2组NIHSS评分、全血纤维蛋白原、血小板黏附率、全血高切黏度及血清内皮素1(ET-1)、血管性血友病因子(vWF)水平均低于治疗前,且B组低于A组;红细胞压积、外周血内皮祖细胞(EPCs)及血清内皮型一氧化氮合酶(e NOS)水平、综合功能评定(FCA)评分、Rivemead运动(RMI指数)、Meyer上肢运功功能指数(S-FMMFA上肢指数)及Meyer下肢运功功能指数(S-FMMFA下肢指数)均高于治疗前,且B组高于A组(P<0.05)。结论扎冲十三味丸有助于改善脑梗死后偏瘫患者血液流变学状态及血管内皮功能,进一步减轻患者神经功能缺损程度,提高运动功能,进而有效提高治疗效果。Objective To investigate the effect of Zhachong Shisanwei Pills on the National Institute of Health Stroke Scale(NIHSS)scores and the upper limb motor function in patients with hemiplegia after cerebral infarction.Methods A total of 150 patients with post-cerebral infarction hemiplegia treated in our hospital from February 2019 to February 2022 were divided into group A and group B by random number table method,with 75 cases in each group.Both groups were given a routine treatment and vinpocetine tablets,while group B was given Zhachong Shisanwei Pills on that basis.Both groups were treated continuously for one month.Results After one month of treatment,the total effective rate of group B was 92.00%,higher than 73.00%of group A(P<0.05).After one month of treatment,the NIHSS scores,the levels of the whole blood fibrinogen,rate of platelet adhesion,whole blood high shear viscosity,serum endothelin 1(ET-1),von Willebrand factor(vWF)of both groups were lower than those before treatment,and group B was lower than group A,while the levels of hematocrit,peripheral blood endothelial progenitor cells(EPCs)and serum endothelial nitric oxide synthase(eNOS),the scores of Functional Comprehensive Assessment(FCA),Rivemead Motion Index(RMI),simple Fugl-Meyer Motor Function Assessment(S-FMMFA upper extremity index)and simple Fugl-Meyer Motor Function Assessment(S-FMMFA lower extremity index)were higher than those before treatment,and group B was higher than group A(P<0.05).Conclusion Zhachong Shisanwei Pills can improve the hemorheology and vascular endothelium function of hemiplegia patients after cerebral infarction,further reduce the degree of neurological defect,and improve the motor function,thereby effectively improving the therapeutic effect.
