扶正化瘀胶囊对乙型肝炎肝硬化患者肝功能及免疫功能的影响

摘　　要：目的观察扶正化瘀胶囊对乙型肝炎肝硬化患者肝功能及免疫功能的影响。方法以2017年10月-2020年10月石家庄市第五医院收治的乙型肝炎肝硬化患者128例为研究对象,采用随机数字表法分为对照组(64例)和治疗组(64例)。对照组采用替诺福韦进行治疗,治疗组在对照组的基础上采用扶正化瘀胶囊进行治疗,2组均治疗48周。结果治疗48周后,治疗组总有效率高于对照组(P<0.05)。2组胁下痞块、胁痛、面色晦暗、乏力及口干不欲饮评分及血清天冬氨酸转氨酶(AST)、丙氨酸转氨酶(ALT)、层粘连蛋白(LN)、透明质酸(HA)、Ⅳ型胶原(Ⅳ-C)水平、瞬时肝弹性F值均低于治疗前,治疗组低于对照组(P<0.05)。2组血清白蛋白(ALB)、血清前白蛋白(PA)水平、外周血CD4^(+)水平及CD4^(+)/CD8^(+)高于治疗前,治疗组高于对照组(P<0.05)。治疗期间,治疗组和对照组总不良反应发生率比较,差异无统计学意义(P>0.05)。结论扶正化瘀胶囊治疗乙型肝炎肝硬化患者,可有效改善患者中医证候,缓解患者肝纤维化,改善患者肝功能,降低患者肝脏硬度,同时调节机体免疫功能,且不会增加不良反应的发生,具有较好的治疗效果。Objective To study the effect of Fuzheng Huayu Capsules on the liver function and immune function in patients with hepatitis B cirrhosis.Methods A total of 128 patients with hepatitis B cirrhosis admitted to the Fifth Hospital of Shijiazhuang from October 2017 to October 2020 were selected as the research subjects and randomly divided into the control group(64 cases)and the research group(64 cases).The control group was treated with tenofovir,while the research group was treated with Fuzheng Huayu Capsules on the basis of that,both for 48 weeks.Results After 48 weeks of treatment,the total effective rate of the research group was higher than that of the control group(P<0.05).The scores of hypocholic mass,hypocholic pain,dark face,fatigue and dry mouth,the levels of serum aspartate aminotransferase(AST),alanine aminotransferase(ALT),laminin(LN),hyaluronic acid(HA),type Ⅳ collagen(Ⅳ-C)and the value of instantaneous liver elasticity F in both groups were lower than before treatment,and the research group was lower than the control group(P<0.05).The levels of serum albumin(ALB),prealbumin(PA),peripheral blood CD4^(+)and CD4^(+)/CD8^(+)in both groups were higher than before treatment,and those in the research group were higher than those in the control group(P<0.05).During the treatment,there was no significant difference in the total incidence of adverse reactions between the research group and the control group(P>0.05).Conclusion In the treatment of patients with hepatitis B cirrhosis,Fuzheng Huayu Capsules can effectively improve the TCM syndromes of patients,relieve the liver fibrosis,improve the liver function,reduce the liver hardness,regulate the immune function,and will not increase the occurrence of adverse reactions,which has a good therapeutic effect.
