摘　　要：恶性胸膜间皮瘤(malignant pleural mesothelioma, MPM)是一种罕见的侵袭性肿瘤。手术是治疗MPM的重要方法之一。但是由于文献报道的过高并发症发生率和围术期死亡比例, 手术的生存获益和适用人群仍然存在争议。本研究将回顾MPM的手术指征, 并讨论和比较以宏观切除(macroscopic complete resection, MCR)为目标的胸膜外全肺切除术(extrapleural pneumonectomy, EPP)和胸膜切除术/剥脱术(pleurectomy/decortication, P/D)在MPM治疗中的作用, 同时展望包括靶向治疗、免疫治疗在内的其他治疗方式的进展。Malignant pleural mesothelioma(MPM)is a kind of rare and aggressive malignant neoplasm.Surgery plays one of the most important roles in the treatment of MPM.However,due to the high morbidity and mortality reported,the survival benefit and indication of surgery are still controversial.This article will review the surgical indications,discuss and compare the roles of extrapleural pneumonectomy(EPP)and pleurectomy/decortication(P/D)which aim to achieve macroscopic complete resection(MCR)in the treatment of MPM.Finally,we summarized the progress of other treatment methods including targeted therapy and immunotherapy.
