改良梭形切口在舌下腺切除术中的应用

摘　　要：目的探讨改良口底黏膜梭形切口在舌下腺切除术中的应用价值,为临床行舌下腺切除并减少术后并发症提供参考.方法回顾性分析43例因舌下腺囊肿采用改良梭形切口行舌下腺切除病例的临床资料,观察此手术入路的特点及患者术后并发症情况.结果所有患者术后均未出现口底及下颌下区肿胀、舌体麻木及创口出血情况,1例患者术后出现创口感染,其余均一期愈合.术后随访1月~5 a,均无复发.结论改良梭形切口操作简单,能够充分显露术区,降低下颌下腺导管及舌神经损伤风险,安全可行.Objective To explore the application of modified fusiform incision in sublingual gland excision,and to provide reference for clinical resection of the sublingual gland and reduce complications.Method Retrospective review of 43 patients with ranula who were resected sublingual gland with the modified fusiform incision was analyzed.The characteristics of the surgical approach and the postoperative complications were observed.Results All patients met the criteria,no postoperatively swelling of oral base and the submandibular area,no tongue hypesthesia or wound bleeding occurred,1 patient occurred postoperative infection,and the rests were healed in one stage.All patients were followed up from 1 month to 5 years,and no recurrence.Conclusion The fusiform incision can give a wider surgical field,simplify the operation.It is safe and feasible to reduce the risk of submandibular gland duct and lingual nerve injury.
