摘 要:目的观察佩戴硬性透气性角膜接触镜(RGPCL)对超高度近视患者视力及视觉相关生活质量(QOL)的影响。方法回顾性病例系列研究。纳入2019年6月至2022年6月郑州大学第一附属医院验配RGPCL的超高度近视患者43例(86只眼),验配RGPCL前进行综合验光,获取其屈光度及框架眼镜最佳矫正视力(BCVA,logMAR),并采用美国眼科学会视功能问卷(NEI-VEQ-25)中文版量表自我评估视觉相关生活质量分析,连续佩戴RGPCL 1个月后,测量患者佩戴RGPCL的视力,并再次使用NEI-VFQ-25问卷进行自我打分,分析佩戴RGPCL对视力及NEI-VFQ-25问卷评分的影响。结果验配RGPCL前框架眼镜BCVA右眼为0.2(0,0.3)、左眼为0.2(0,0.4),佩戴RGPCL 1个月时RGPCL戴镜视力右眼为0.1(0,0.2)、左眼为0(-0.1,0.3),双眼RGPCL戴镜视力与框架眼镜BCVA相比有显著提高,差异有统计学意义(Z=-5.06、-5.21,均P<0.001);佩戴RGPCL 1个月时NEI-VFQ-25问卷评分与佩戴RGPCL前比较,在整体健康状况(Z=-2.00,P=0.046)、总体视力(Z=-5.89,P<0.001)、近距离活动(Z=-4.14,P<0.001)、远距离活动(Z=-5.32,P<0.001)、周边视力(Z=-5.23,P<0.001)、社会功能(Z=-3.90,P<0.001)、精神健康状况(Z=-5.46,P<0.001)、社会角色限制(Z=-4.45,P<0.001)、依赖程度(Z=-5.40,P<0.001)、驾车(Z=-4.66,P<0.001)、色觉(Z=-3.13,P=0.002)等11个维度上均有提高,差异有统计学意义(均P<0.05)。在维度"眼痛"中,差异无统计学意义(Z=-0.02,P=0.986)。结论RGPCL可以帮助超高度近视患者提高矫正视力,矫正视力优于框架眼镜,并提高其视觉相关生活质量。Objective To observe the effects of rigid gas permeable contact lens(RGPCL)on visual acuity and vision-related quality of life(QOL)in patients with super high myopia.Methods This was a retrospective case series study.A total of 86 eyes of 43 patients with super high myopia who were fitted with RGPCL from Jun.2019 to Jun.2022 in the First Affiliated Hospital of Zhengzhou University were included.All cases underwent comprehensive refraction before fitting RGPCL to obtain the diopter and best corrected visual acuity(BCVA,logMAR)of frame glasses.The Chinese version of the National Eye Institute Visual Function Questionnaires(NEI-VEQ-25)to self-score for visual-related quality of life analysis were used.After wearing RGPCL for 1 month,the visual acuity of the RGPCL was measured,and the NEI-VFQ-25 questionnaire was used for self-scoring.The effect of wearing RGPCL on visual acuity and NEI-VFQ-25 questionnaire score were analyzed.Results The BCVA of RGPCL(pre-frame glasses)was 0.2(0,0.3)of the right eyes(43 eyes)and 0.2(0,0.4)of the left eyes(43 eyes).When RGPCL was worn for 1 month,the visual acuity was 0.1(0,0.2)of the right eyes and 0(-0.1,0.3)of the left eyes.The visual acuity of RGPCL in the right and left eyes was significantly improved compared with the BCVA with glasses,and the difference was statistically significant(Z=-5.06,-5.21;all P<0.001).Compared with before wearing RGPCL,the NEI-VFQ-25 questionnaire scores improved in the following 11 items when wearing RGPCL for 1 month:the general health(Z=-2.00,P=0.046),general vision(Z=-5.89,P<0.001),near activities(Z=-4.14,P<0.001),distance activities(Z=-5.32,P<0.001),peripheral vision(Z=-5.23,P<0.001),social functioning(Z=-3.90,P<0.001),mental health(Z=-5.46,P<0.001),role difficulties(Z=-4.45,P<0.001),dependency(Z=-5.40,P<0.001),driving(Z=-4.66,P<0.001)and color vision(Z=-3.13,P=0.002),and the differences were statistically significant(all P<0.05).In the item"ocular pain",the difference was not statistically significant(Z=-0.02,P=0.986).Conclusion RGPCL can help pa
