摘 要:目的:探讨渗透树脂与微研磨技术用于修复正畸后牙面白斑样病损的美学效果。方法:选取2015年4月-2020年7月笔者医院就诊的60例正畸后牙面白斑样病损患者,随机分为治疗组(30例)和对照组(30例),对照组给予微研磨技术治疗,治疗组给予渗透树脂技术治疗。比较两组修复后患牙的临床疗效,治疗前后病损面积比和治疗后有效率,治疗前后软件评分、主观评分、表面显微硬度(SMH)值和L*值,以及治疗后不良反应发生情况。结果:治疗后,治疗组优良率为90.00%,高于对照组的66.67%(P<0.05);两组患牙病损面积低于治疗前,且治疗组病损面积比低于对照组,有效率高于对照组,差异有统计学意义(P<0.05);两组软件评分和主观评分高于治疗前,且治疗组软件评分和主观评分高于对照组,差异均有统计学意义(P<0.05);治疗后,两组SMH高于治疗前,L*值低于治疗前,且治疗组SMH高于对照组,L*值低于对照组,差异均有统计学意义(P<0.05);治疗后,两组均未发生明显的不良反应。结论:治疗白斑样病损,相比微研磨技术,渗透树脂治疗美学效果更好,可以减少病损面积,改善牙釉质颜色和硬度,安全性好。Objective To explore the cosmetic effect of infiltrating resin and micro-grinding technology in the repair of dental white spot lesions after orthodontics. Methods A total of 60 patients with dental white spot lesions after orthodontics in the hospital were enrolled between April 2015 and July 2020. They wee randomly divided into treatment group(30 cases) and control group(30 cases). The control group was treated with micro-grinding technology, while treatment group was treated with infiltrating resin. The clinical curative effect after treatment, ratio of lesion area before and after treatment, response rate after treatment, software score, subjective score, surface microhardness(SMH) value and L* value before and after treatment, and occurrence of adverse reactions after treatment were compared between the two groups. Results After treatment, good rate of treatment group was higher than that of control group(90.00% vs 66.67%)(P<0.05). After treatment, lesion area of offending teeth in both groups was reduced. The ratio of lesion area in treatment group was lower than that in control group, and response rate was higher than that in control group(P<0.05). After treatment, software score and subjective score in both groups were increased, which were higher in treatment group than control group,the differences were statistically significant(P<0.05). After treatment, SMH was increased, while L* was decreased in both groups(P<0.05). SMH in treatment group was greater than that in control group, while L* was lower than that in control group,the differences were statistically significant(P<0.05). After treatment, there were no obvious adverse reactions in either group. Conclusion Compared with micro-grinding technology,cosmetic effect of infiltrating resin is better in the treatment of dental white spot lesions, which can reduce lesion area, improve color and hardness of dentin, with high safety.
