牙根十字分牙法联合医用胶原蛋白海绵在下颌阻生齿拔除术中的应用效果观察

摘　　要：目的:探讨牙根十字分牙法联合医用胶原蛋白海绵拔除下颌阻生齿的疗效。方法:选取2019年1月-2021年1月在笔者医院就诊的112例下颌阻生齿患者,根据患者病历号末尾奇偶数分为观察组(n=56)和对照组(n=56),对照组给予牙根十字分牙法拔牙,观察组在对照组基础上加用医用胶原蛋白海绵,观察两组拔牙时间、术中出血量、术后疼痛程度、肿胀程度及生活质量等。结果:两组拔牙时间、术中出血量比较差异无统计学意义(P>0.05);观察组术后1 d疼痛视觉模拟评分(Visual analogue scale,VAS)和术侧口角至耳屏最下点的体表距离(Co-T)分别为(2.05±0.65)分和(10.40±0.80)mm,明显低于对照组,差异有统计学意义(P<0.05);观察组术后1 d张口受限程度较对照组轻(P<0.05),其0度比例为85.71%;观察组术后1周症状严重度量表(PoSSe)肿胀、疼痛和总分分别为(2.51±0.87)分、(3.12±0.97)分、(17.01±2.15)分,明显低于对照组(P<0.05);观察组术后1 d龈沟液白细胞介素-6(IL-6)、IL-1β分别为(5.50±0.84)ng/ml和(2.10±0.65)ng/ml,明显低于对照组(P<0.05),观察组干槽症发生率明显低于对照组,差异均有统计学意义(P<0.05)。结论:牙根十字分牙法联合医用胶原蛋白海绵用于下颌阻生齿拔除术中有较好效果,可改善患者术后疼痛、肿胀等症状,并提高其生活质量。Objective To investigate the effect of root cross method combined with medical collagen sponge in the extraction of mandibular impacted teeth. Methods 112 patients with mandibular impacted teeth in author’s hospital from January 2019to January 2021 were selected and divided into the observation group(n = 56) and the control group(n = 56) according to the parity at the end of the patient’s medical record number, the control group was treated with root cross tooth extraction, and the observation group was treated with medical collagen sponge on the basis of the control group, the extraction time, intraoperative blood loss, postoperative pain, swelling and quality of life were observed. Results There was no significant difference in tooth extraction time and intraoperative blood loss between the observation group and the control group(P>0.05). The visual analogue scale(VAS) and the body surface distance from the operative corner to the lowest point of the tragus(CO-T) in the observation group at 1 day after operation were(2.05±0.65) and(10.40±0.80) mm, which were significantly lower than those in the control group(P<0.05). The degree of mouth opening limitation in the observation group on day 1 after surgery was less than that in the control group(P<0.05), and the proportion of 0 degree was 85.71%. The symptom severity scale(PoSSe)swelling, pain and total scores in observation group were(2.51±0.87),(3.12±0.97) and(17.01±2.15) points, respectively,which were significantly lower than those in control group(P<0.05). The Interleukin-6(IL-6) and IL-1β in gingival creval fluid of observation group on day 1 after surgery were(5.50±0.84) ng/ml and(2.10±0.65) ng/ml, respectively, which were significantly lower than those of control group(P<0.05). The incidence of dry slot in the observation group was 3.57%, which was significantly lower than that in the control group(P<0.05).
