摘  要:传统肝脏分段解剖方法是目前肝脏恶性肿瘤切除技术的理论基础。随着对肝脏解剖学理解的加深,肝脏动态流域学说的提出将肝脏解剖方法从传统的静态树干型向动态流域型转变。在肝脏动态流域学说指导下,笔者提出肝脏恶性肿瘤靶域切除技术,该技术有别于解剖性或非解剖性肝切除,不强调按照人为设定的肝段解剖进行切除,而是以病灶和可能存在浸润和转移流域为导向,结合肝脏恶性肿瘤的生物学特性,决定手术切除靶域。The conventional liver anatomy is the theoretical basis of the current techniques for resection of liver malignant neoplasms. With the deepening understanding of liver anatomy, the proposal of liver dynamic watershed theory has transformed the liver anatomy method from the traditional static trunk type to the dynamic watershed type. Under the guidance of dynamic liver blood watershed theory, the author proposes the target-territory resection techniques for liver malignant neoplasms, which is different from anatomical or non-anatomical liver resection and does not emphasize the resection according to the artificially preset liver segments. Instead, it takes the lesion and possible infiltration and metastasis watershed as the guide to determine the targetterritory for resection by combining the biological characteristics of liver malignant neoplasms.
