经鼻内镜下眼眶减压术治疗中重度甲状腺相关眼病疗效观察

摘　　要：目的探讨经鼻内镜下眼眶减压术治疗中重度甲状腺相关眼病的治疗效果。方法选取我院在2015年6月至2021年5月期间收治的60例中重度甲状腺相关眼病患者为研究对象,所有患者均行经鼻内镜下眼眶减压术,对比患者治疗前和治疗3个月后的最佳矫正视力、眼内压、睑裂高度和眼球突出度。结果60例87眼中有50只眼(57.5%)术后最佳矫正视力提高,术前术后对比有显著性差异(U=319.7,P=0.04),其余较为稳定,无术后视力持续下降者,术后3月眼球突出度、眼内压和睑裂高度的改善均有显著性差异(t=16.56,P=0.00;t=38.12,P=0.00;t=12.67,P=0.00)。结论经鼻内镜下眼眶减压术治疗重度甲状腺相关眼病可有效改善患者视力,降低眼内压,利于眼球回退,改善眼部症状。Objective To explore the therapeutic effect of orbital decompression under nasal endoscopy in the treatment of moderate to severe thyroid associated ophthalmopathy.Methods 60 patients with moderate to severe thyroid associated ophthalmopathy who were admitted to our hospital from June 2015 to May 2021 were selected as the research objects.All patients underwent transnasal endoscopic orbital decompression.Best corrected visual acuity,intraocular pressure,palpebral fissure height and eyeball protrusion were evaluated in all patients before and 3 months after operation.Results Best corrected visual acuity was improved in 50(57.5%) of 87 eyes in 60 cases with a significant difference between the preoperative and postoperative(U=319.7,P=0.04).The rest were relatively stable and there was no continuous decrease in visual acuity after operation.It proved to be significant different in the improvement of eyeball protrusion,intraocular pressure and palpebral fissure height 3 months after operation(t=16.56,P=0.00;t=38.12,P=0.00;t=12.67,P=0.00).Conclusion Endoscopic orbital decompr ession for severe thyroid-related ophthalmopathy can effectively improve patients’ visual acuity,reduce intraocular pressure,facilitate eyeball regression and improve eye symptoms.
