Sutureless Nonphacoemulsification Combined with Posterior Segment Infusion for the Treatment of High Myopia with Black Nucleus Cataract after Vitrectomy

摘　　要：目的 探讨玻璃体切除术后高度近视合并黑核白内障的手术方法和术中技巧。方法 对2016年3月-2021年11月收治的11例玻璃体切除术后高度近视合并黑核白内障,在平坦部玻璃体腔灌注下采用无缝线非超声乳化术行白内障囊外摘除及人工晶体植入术。结果 所有手术均获成功,术后矫正视力较术前矫正视力提高3行1例,提高2行4例,提高1行5例,1例黄斑病变严重者视力无变化。术中植入囊袋张力环2例,后囊膜破裂1例,术后角膜水肿7例,2-3天后逐步消退。结论 在玻璃体腔灌注下采用无缝线非超声乳化术治疗玻璃体切除术后高度近视合并黑核白内障安全有效,注意细节操作是手术成功的关键。Objective To investigate the surgical methods and intraoperative skills of high myopia with black nucleus cataract after vitrectomy. Methods From March 2016 to November 2021, 11 cases of high myopia with black nucleus cataract after vitrectomy were treated with sutureless non-phacoemulsification cataract extraction and intraocular lens implantation under pars plana intravitreal perfusion. Results All the operations were successful. The corrected visual acuity was improved after operation, 3 lines in 1 case, 2 lines in 4 cases, 1 line in 5 cases, and no change in 1 case due to severe maculopathy. Capsular tension ring was implanted in 2 cases and posterior capsule rupture in 1 case. Postoperative corneal edema occurred in 7 cases and gradually subsided after two to three days. Conclusions Sutureless non-phacoemulsification under intravitreal perfusion is safe and effective in the treatment of high myopia with black nucleus cataract after vitrectomy. Attention to detailed operation is the key to successful operation.
