摘  要:目的:探究脑后循环缺血应用CT灌注的诊断价值。方法:回顾分析韶关市第三人民医院和粤北人民医院2020年6月至2021年6月期间进行诊治120例后循环缺血患者的临床资料,全部经临床检查确诊,对比正常脑组织和灌注缺损区的最大峰值时间(TTP)、平均通过时间(MTT)、局部脑血容量(rCBV)、局部脑血流量(rCBF)。结果:本次研究120例患者中,CT灌注检出病灶184个,其中Ⅰ期79个、Ⅱ期65个、Ⅲ期40个。后循环各部位不同分期脑组织的rCBF、rCBV、MTT、TTP差异于正常组织(P<0.05)。结论:CT灌注在后循环缺血诊断中的应用价值显著,可根据灌注参数鉴别后循环各部位的缺血、梗死和正常脑组织。Objective:To explore the diagnostic value of CT perfusion in cerebral circulation ischemia.Methods:The clinical data of 120 patients with posterior circulation ischemia treated in Shaoguan Third People’s Hospital and Yuebei People’s Hospital from June 2020 to June 2021 were analyzed retrospectively. All of them were confirmed by clinical examination. The maximum peak time(TTP), mean transit time(MTT), regional cerebral blood volume(rCBV) and regional cerebral blood flow(rCBF) of normal brain tissue and perfusion defect area were compared.Results:Among 120 patients in this study, 184 lesions were detected by CT perfusion, including 79 lesions in stage I, 65 lesions in stage II and 40 lesions in stage III. The rCBF, rCBV, MTT and TTP of brain tissue in different stages of posterior circulation were different from those of normal tissues(P<0.05).Conclusion:The application value of CT perfusion in the diagnosis of posterior circulation ischemia is significant. Ischemia, infarction and normal brain tissue in different parts of posterior circulation can be distinguished according to perfusion parameters.
