YKL-40与HE4联合ROMA指数鉴别卵巢肿瘤良恶性的诊断价值

摘　　要：目的:探讨血清人类软骨糖蛋白39(YKL-40)、附睾蛋白4(HE4)、卵巢癌风险预测指数(ROMA)诊断卵巢肿瘤良恶性的临床价值。方法:100例卵巢癌患者设为恶性组,100例卵巢良性肿瘤患者设为良性组;检测并对比两组患者的血清YKL-40、HE4水平、ROMA指数;分析不同病理学分期(FIGO)的卵巢癌患者YKL-40、HE4、ROMA指数差异,并以病理学结果作为判断金标准计算各项指标单独或联合应用鉴别诊断卵巢癌良恶性的价值。结果:恶性组患者的YKL-40、HE4、ROMA指数均高于良性组(P<0.05);Ⅰ期和Ⅱ期(FIGO分期)的卵巢癌患者的YKL-40、HE4、ROMA指数均低于Ⅲ期和Ⅳ期(FIGO分期)(P<0.05);YKL-40、HE4、ROMA指数诊断卵巢癌与卵巢良性疾病的灵敏度分别为42.00%、47.00%、66.00%,特异度分别为93.00%、83.00%、77.00%;YKL-40+HE4+ROMA指数三者联合诊断卵巢癌与卵巢良性疾病的灵敏度为89.00%,特异度为86.00%。结论:YKL-40、HE4、ROMA指数三者联合诊断卵巢癌与卵巢良性疾病的灵敏度高于三者单独检测,具有较高的临床价值。Objective:To investigate the value of serum human cartilage glycoprotein 39(YKL-40),epididymal protein 4(HE4) and ovarian cancer risk prediction index(ROMA) in differential diagnosis of ovarian tumor properties.Methods:100 ovarian cancer patients were set as malignant group, and 100 ovarian benign tumor patients were set as benign group.The serum YKL-40,HE4 levels and the ROMA index of the two groups of patients were detected and compared.The difference of YKL-40,HE4,and ROMA indexes in ovarian cancer patients with FIGO were analyzed, and the pathological results were used as the gold standard to calculate the value of each index used alone or in combination in the differential diagnosis of benign and malignant ovarian cancer.Results:The YKL-40,HE4 and ROMA indexes of the malignant group were higher than those of the benign group(P<0.05).The YKL-40,HE4 and ROMA indexes of ovarian cancer patients with stage I and II(FIGO stage) were lower than those in stage III and IV(FIGO staging)(P<0.05),YKL-40 had a sensitivity of 42.00% and a specificity of 93.00% in the differential diagnosis of ovarian cancer and ovarian benign diseases, the sensitivity of HE4 in the differential diagnosis of ovarian cancer and ovarian benign diseases was 47.00%,specificity was 83.00%.ROMA index had a sensitivity of 66.00% and a specificity of 77.00% in the differential diagnosis of ovarian cancer and ovarian benign diseases, the sensitivity of YKL-40+HE4+ROMA index in the differential diagnosis of ovarian cancer and ovarian benign diseases was 89.00%,specificity was 86.00%.Conclusion:YKL-40 and HE4 combined with ROMA index in the differential diagnosis of ovarian cancer and ovarian benign diseasesis higher than that of the three alone, and it has high clinical value.
