超声引导下肝脏穿刺活检对乙型肝炎肝硬化的应用价值

摘　　要：目的 探讨超声引导下经皮肝脏穿刺活检的临床应用及应用价值。方法 选择泰安八十八医院接受肝脏穿刺活检的225例乙型肝炎肝硬化患者作为研究对象,依次对患者实施瞬时弹性成像(FibroScan)检查和超声引导下肝脏穿刺活检,分析超声引导下肝脏穿刺活检的并发症发生率及穿刺后疼痛评分,比较FibroScan检查和超声引导下肝脏穿刺活检对不同肝纤维化分期的诊断符合率。结果 超声引导下肝脏穿刺活检的并发症发生率为1.33%,其中感染发生率为0.89%,胆瘘发生率为0.44%,穿刺后疼痛评分为(3.30±0.52)分。超声引导下肝脏穿刺活检对肝纤维化S1期、S2期、S3期、S4期的诊断符合率均高于FibroScan检查(P<0.05)。结论 超声引导下肝脏穿刺活检在乙型肝炎肝硬化诊断中具有显著的应用价值,可提高对肝纤维化程度诊断效能。Objective To explore the clinical application value of ultrasound-guided liver biopsy. Methods 225 cases of patients with hepatitis B cirrhosis who underwent liver biopsy in Taian 88th Hospital were selected as the research objects. The patients were successively subjected to FibroScan and ultrasound-guided liver biopsy. The incidence of complications and pain score after liver biopsy were analyzed, and the diagnostic accuracy of FibroScan and ultrasound-guided liver biopsy in different stages of liver fibrosis were calculated and compared. Results The incidence of complication of ultrasound-guided liver biopsy was 1.33%, including the incidence of infection was 0.89%, the incidence of biliary fistula was 0.44%, the pain score after liver biopsy was(3.30±0.52) scores. The diagnostic coincidence rate of ultrasound-guided liver biopsy in S1, S2, S3and S4stages of liver fibrosis was higher than that of FibroScan(P<0.05). Conclusion Ultrasound-guided liver biopsy has significant application value in the diagnosis of hepatitis B cirrhosis, which can improve the efficiency of diagnosis of liver fibrosis.
