上颌窦底剩余骨高度小于4 mm的种植外科方案及适应症探讨

摘　　要：目的 探讨上颌窦底剩余骨高度小于4 mm的牙槽嵴种植外科方案。方法 回顾2014年7月至2020年7月上颌窦提升术病例,测量患者CBCT,归纳上颌后牙区剩余骨高度小于4 mm时的种植外科方案。对4 mm≤RBH <10 mm和RBH <4 mm的种植体脱落率及并发症进行统计分析。结果 共纳入种植体409颗,其中上颌后牙区剩余骨高度小于4mm的种植体127颗。根据牙槽嵴特征将上颌窦底剩余骨高度小于4 mm的牙槽嵴种植外科方案归纳为A、B1、B2、C四类,术后1~5年种植体累计脱落率为4.72%,总计发生各类并发症19例。结论将上颌后牙区牙槽嵴特征与对应外科方案归纳成图表,为RBH小于4 mm牙槽嵴的种植外科方案选择提供参考。Objective To investigate the surgical procedures for severe bone atrophy(RBH < 4 mm) in the maxillary posterior area. Methods Patients who underwent sinus floor elevation were collected. The surgical scheme for the residual bone height less than 4 mm was summarized. The implant loss rate and complications were compared. Results A total of 409implants were included, including 127 implants with RBH less than 4mm at the implant site. The clinical procedures were divided into four categories(A,B1,B2,C). The cumulative failure rate of implants 1-5 years after implant insertion was 4.72%,and complications occurred in 19 cases. Conclusions The present study summarizes the surgical procedures for implant sites with RBH less than 4 mm in a diagram and provides a reference for the selection of relevant surgical procedures.
