健康中国视域下第一代农民工健康医疗可及性析论——基于30位第一代农民工的质性研究

摘　　要：运用深度访谈等定性研究方法对第一代农民工的健康及医疗服务可及性影响因素进行分析,结果显示,第一代农民工存在健康的弱势累积现象,其医疗健康服务可及性较差,这些都导致他们的健康由累积的弱势演变为累积的风险。新农合政策对第一代农民工的健康医疗服务可及性存在时空滞后问题,其低便携性抑制了农民工的健康维护能力,放大了他们的健康风险,并为其晚年生活留下健康隐患。健康中国建设实质在于落实人人享有医保政策目标,第一代农民工群体的健康医疗可及性问题实质在于医保政策的属地原则与农民工高流动性存在二元张力,消除此二元张力,是解决农民工健康弱势累积和增强其医疗服务可及性的核心举措,也是健康中国“2030”规划纲要建设的重要内容。Using qualitative research approaches such as in-depth interviews,this paper analyses the factors influencing the health and accessibility of health services for first-generation migrant workers.The research findings are:The cumulative health disadvantage of first-generation migrant workers and the poor accessibility of their health services have led to their health evolving from cumulative disadvantage to cumulative risk.There is a time and space lag in the accessibility of health care services for the first generation of migrant workers,and the low portability of the NRCMS(New Rural Cooperative Medical System)inhibits migrant workers'ability to maintain their health,magnifies their health risks,and leaves health hazards for their later life.The essence of the construction of a healthy China lies in the implementation of a health insurance policy for all.The problem of health care accessibility for the first generation of migrant workers lies in the dualistic tension between the territorial principle of the health insurance policy and the high mobility of migrant workers,and the elimination of the dualistic tension between the existing health insurance policy and the high mobility of migrant workers is the core measure to solve the accumulation of health disadvantages of migrant workers and enhance their access to health care services,which is also an important element of the construction of the"Healthy China 2030"plan.
